Efficacy of metoprolol therapy is proven in randomized clinical studies in patients with angina and myocardial infarction (MI). In angina patients metoprolol prevents cardiac attacks, MI, reduces nitroglycerine consumption, increases exercise tolerability, prolongs the exercise time before ST segment depression (succinate better than tartrate), decrease of angina intensity. In MI patients metoprolol therapy reduces mortality, sudden death, recurring MI and the rate of early post MI angina attacks. Nowadays metoprolol is the only β-blocker having indication on secondary MI prevention. Besides for the present metoprolol succinate is the only β-blocker with proven direct antisclerosis effect. According to Swedish clinicoeconomical study in patients after MI secondary prevention with metoprolol therapy saves the costs in comparison with placebo. American clinicoeconomical model of metoprolol and atenolol usage in all patients with MI could result in significant reduction in mortality and recurring MI rate, prolong the life and improve its quality, save financial resources. The cost of monthly treatment of angina patient with metoprolol tartrate (Betaloc) and metoprolol succinate (Betaloc ZOK) is 135 and 354 rubles, respectively. The price range of comparative β-blockers in ascending order is the following: atenolol (Atenolol Nicomed) → metoprolol tartrate (Betaloc) → metoprolol succinate (Betaloc ZOK) → bisoprolol (Concor) → nebivolol (Nebilet). In conclusion, metoprolol therapy is the one of mostly economically reasonable approach.</p></abstract><trans-abstract xml:lang="ru"><p>Представлен обзор клинических и клинико-экономических исследований, а также результаты собственного сравнительного анализа стоимости применения метопролола тартрата (Беталок) и метопролола сукцината (Беталок ЗОК) у пациентов с ишемической болезнью сердца. Эффективность метопролола в лечении пациентов со стенокардией и инфарктом миокарда (ИМ) доказана в качественных клинических исследованиях. Метопролол в терапии пациентов со стенокардией способен предупреждать рецидивирование ангинозных приступов, ИМ, снижает потребность в нитроглицерине, способствует повышению толерантности к физической нагрузке, увеличивает время до появления ишемических изменений сегмента ST (в большей степени метопролола сукцинат), снижает интенсивность ангинозных болей. У пациентов с ИМ терапия метопрололом способствует снижению летальности, внезапной смерти, повторных ИМ, частоты ранней постинфарктной стенокардии. Метопролол сегодня является единственным кардиоселективным блокатором, имеющим показания к применению при вторичной профилактике ИМ. Кроме того, метопролола сукцинат является пока единственным препаратом из группы β-блокаторов, для которого доказано наличие антиатеросклеротического эффекта. Согласно данным шведского клинико-экономического исследования, применение метопролола с целью вторичной профилактики у пациентов, перенесших ИМ, приводит к сбережению затрат по сравнению с плацебо. Разработанная американскими исследователями клинико-экономическая модель применения метопролола и атенолола у всех пациентов (американская популяция) с ИМ, не имеющих абсолютных противопоказаний к назначению β-блокаторов, может привести к 2020 году к значительному снижению показателей смертности, предотвращению развития повторных ИМ, увеличению количества лет качественной жизни, а также к потенциальному сбережению финансовых ресурсов. Стоимость месяца терапии пациента со стенокардией метопрололом тартратом (Беталок) и метопрололом сукцинатом (Беталок ЗОК) в средних терапевтических дозировках обходится в 135 руб. и 354 руб., соответственно.
