В настоящее время отсутствуют общепринятые рекомендации по применению ривароксабана совместно с амиодароном, верапамилом или дилтиаземом у пациентов с клиренсом креатинина (КлКр) <80 мл/мин. По мнению ряда исследователей, при почечной недостаточности амиодарон, верапамил и дилтиазем способствуют существенному повышению плазменной концентрации ривароксабана, что сопровождается увеличением риска кровотечений. Согласно предварительным расчетам, представляется целесообразным при совместном приеме с данными препаратами снижать дозу ривароксабана до 15 мг/сут у пациентов с КлКр 50-79 мл/мин и до 10 мг/сут у пациентов с КлКр <50 мл/мин.
