The classification of the Tn-tests depending on their sensitivity is presented. The possible reasons of troponins appearance in blood of healthy people are shown. Authors consider a 3-hour algorithm for myocardial infarction (MI) diagnostic, recommended by the expert group in 2012. Study results of 2011-2015 years are presented as the basis for the development of a one-hour MI diagnostic algorithm, recommended by the European Society of Cardiology in 2015. Authors discuss the results of studies showing that modern HS-cTnt tests (together with ECG assessment) are capable to diagnose MI in the early stages. They significantly increase the number of identified MI, especially MI without ST segment elevation, as well as identify the group of patients with subsequent favorable prognosis.</p></abstract><trans-abstract xml:lang="ru"><p>Обзор литературы посвящен значению использования высокочувствительных тестов на тропонин (hs-cTn) в диагностике острого коронарного синдрома. Представлена классификация Tn-тестов  в зависимости  от степени их чувствительности. Приводятся  возможные причины появления тропонина в крови у здоровых лиц. Изложен 3-х часовой алгоритм диагностики инфаркта миокарда (ИМ), рекомендованный группой экспертов в 2012  г. Представлены результаты исследований 2011 – 2015 гг., послужившие основанием для разработки одночасового алгоритма диагностики ИМ, рекомендованного к использованию в клинической практике Европейским обществом кардиологов в 2015 г. Приведены результаты исследований, свидетельствующие, что современные hs-cTn тесты (совместно с оценкой ЭКГ) способны диагностировать ИМ уже на ранних стадиях, значительно увеличивают число выявляемых ИМ, особенно за счет ИМ без подъема сегмента ST, а также идентифицируют группу лиц с последующим благоприятным прогнозом.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>острый коронарный синдром</kwd><kwd>высокочувствительные  тесты на тропонин</kwd><kwd>диагностика инфаркта миокарда</kwd></kwd-group><kwd-group xml:lang="en"><kwd>acute coronary syndromes</kwd><kwd>highly sensitive troponin (hs-cTn) tests</kwd><kwd>myocardial infarction diagnostics</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">The Joint European Society of Cardiology/American College of Cardiology Committee.
