To study the effect of transcatheter aortic valve implantation (TAVI), performed by different types of prostheses and various surgical access, on the prognosis of patients with critical aortic stenosis and comorbidities.</p></sec><sec><title>Material and methods</title><p>Material and methods. Patients (n=130) that had consistently performed 80 TAVI by Edwards valve transfemoral (n=50) and transapical (n=30) access, as well as 50 transcatheter aortic valve replacement by CoreValve system were included into the study. Complications including perioperative mortality, total 30-day mortality, as well as post-hospital mortality were registered during aortic valve replacement, immediately after surgery, before the expiry of 30 days. Mean follow-up was 2.2 years (range 0.2 to 5.2 years).</p></sec><sec><title>Results</title><p>Results. Hospital mortality was on average 6.9%. 121 patients had been discharged from the department after the surgery. The number of deaths in the post-hospital period was 14.8%. Valve type and the type of access had no effect on post-hospital mortality. Men died more than 2.5 times often than women, regardless of age. Atrioventricular block, pacemaker implantation, and history of chronic obstructive pulmonary disease were the most significant prognostic factors. An important role of minor stroke and renal failure should be noted. Mortality did not depend on the surgical access or valve type. All parameters characterizing the intervention were significantly associated with mortality, both during and after surgery. The proportion of survivors at the end of the first year of observation using Corvalve system was 86.9%, Edwards valve by transfemoral access - 88% and Edwards valve by transapical access – 85.4% (insignificant differences for all groups, p&gt;0.05). Two-year survival was 77.5%, 82.5% and 82.7%, respectively (also insignificant differences for all groups, p&gt;0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. TAVI is the method of choice, reasonable alternative approach for surgical valve replacement in patients with high surgical risk, although mortality after TAVI is still quite high and amounted to 6.9% in our study. It makes it possible to extend life and improve its quality that is of great social importance in the global trend of an aging population.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель. Изучить влияние транскатетерной имплантации аортального клапана (ТИАК), выполненной с помощью различных типов протезов и разных вариантов хирургических доступов, на прогноз пациентов с критическим аортальным стенозом и сопутствующей патологией.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 130 пациентов, которым последовательно выполнено 80 операций ТИАК с использованием баллон-расширяемых клапанов Edwards трансфеморальным (n=50) и трансапикальным (n=30) доступами, а также 50 протезирований аортального клапана саморасширяемыми клапанами CoreValve. Во время протезирования аортального клапана, сразу после операции, до истечения 30-дней регистрировались осложнения, в том числе периоперационная летальность, общая 30-дневная летальность, а также послегоспитальная смертность. Среднее время наблюдения составило 2,2 лет (от 0,2 до 5,2 лет).</p></sec><sec><title>Результаты</title><p>Результаты. Госпитальная летальность в среднем составила 6,9%. После операции 121 больной был выписан из отделения. В послегоспитальном периоде число умерших составило 14,8%. Вид клапана и тип доступа не влияли на послегоспитальную смертность, мужчины умирали более чем в 2,5 раза чаще женщин независимо от возраста. Из общего числа показателей, включенных в анализ, наиболее значимыми для прогноза оказались атриовентрикулярная блокада, имплантация электрокардиостимулятора и наличие хронической обструктивной болезни легких в анамнезе. Необходимо отметить немаловажную роль малого инсульта и почечной недостаточности. При анализе прогноза жизни больных, перенесших операцию ТИАК в раннем послегоспитальном периоде, а также в течение первых двух лет было показано, что смертность не зависела ни от доступа, ни от типа клапана. При этом практически все показатели, характеризующие вмешательство, значимо взаимосвязаны со смертностью, как во время, так и после операции. К концу первого года наблюдения доля выживших при использовании клапана Corvalve составила 86,9%, клапана Edwards с трансфеморальным доступом – 88%, и клапана Edwards с трансапикальным доступом – 85,4%, различия между группами статистически незначимы. Двухлетняя выживаемость составила 77,5%, 82,5% и 82,7%, соответственно, что также статистически незначимо.</p></sec><sec><title>Заключение</title><p>Заключение: Несмотря на то, что летальность после операций ТИАК все еще довольно высока, и в нашем исследовании составила 6,9% госпитальная (и 30-дневная), данная методика является операцией выбора, обоснованным альтернативным подходом хирургической замене клапана у больных высокого хирургического риска, что позволяет продлить и улучшить качество жизни, и что при мировой тенденции постарения населения имеет важнейшее общественное значение.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>транскатетерная имплантация аортального клапана</kwd><kwd>общая летальность</kwd><kwd>послегоспитальная летальность</kwd><kwd>осложнения</kwd></kwd-group><kwd-group xml:lang="en"><kwd>transcatheter aortic valve implantation</kwd><kwd>total mortality</kwd><kwd>post-hospital mortality</kwd><kwd>complications</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Rosenhek R., Binder T., Porenta G., et al.
