Levosimendan infusion can be used in the treatment of patients with acute decompensated heart failure (ADHF) with a reduction in cardiac output and signs of severe congestion/pulmonary edema.</p></sec><sec><title>Aim</title><p>Aim. To study impact of levosimendan on renal function in patients with ADHF with reduced systolic function.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study was a prospective, randomized trial. We enrolled 30 men (age 62.5 [55.8-69.3] years) hospitalized with ADHF with reduced systolic function (left ventricular ejection fraction &lt;40%), increased level of brain natriuretic peptide (BNP&gt;500 pg/mL) and systolic blood pressure &gt;125 mmHg. All patients were randomized into 2 groups of 15 people each. In the first group, the patients received an intravenous infusion of levosimendan 0.1 μg/kg/min for 24 hour added to standard therapy. The second group received standard therapy.</p></sec><sec><title>Results</title><p>Results. 24-hour levosimendan infusion significantly increased the glomerular filtration rate levels from 65.4 [45.2-99.2] mL/min/1.73m2 at baseline to 79.0 [66.3-93.1] mL/min/1.73m2 at discharge (p= 0.011), greatly decreased serum creatinine from 1.17 [0.90-1.55] mg/dL at baseline to 1.01 [0.89-1.14] mg/dL at discharge (p = 0.009) and blood urea nitrogen and at the same time improved renal blood flow in patients with ADHF while there were no clinically significant changes in the studied parameters in the standard therapy group.</p></sec><sec><title>Conclusion</title><p>Conclusion. Levosimendan had a positive effect on renal function in patients with ADHF with reduced systolic function.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Актуальность</title><p>Актуальность. Инфузия левосимендана может быть использована при лечении пациентов с острой декомпенсацией сердечной недостаточности (ОДСН) со сниженным сердечным выбросом и признаками выраженного застоя/отека легких.</p></sec><sec><title>Цель</title><p>Цель. Изучить влияние левосимендана на функцию почек у пациентов с ОДСН со сниженной систолической функцией.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В проспективное рандомизированное исследование было включено 30 мужчин, госпитализированных с ОДСН (возраст 62,5 [55,8; 69,3] лет) со сниженной систолической функцией (фракция выброса левого желудочка &lt;40%), повышенным уровнем мозгового натрийуретического пептида (BNP&gt;500 пг/мл) и систолическим артериальным давлением &gt;125 мм рт. ст. Все пациенты были рандомизированы на 2 группы по 15 человек в каждой. В первой группе в дополнение к стандартной терапии пациентам проводилась внутривенная 24-часовая инфузия левосимендана в дозе 0,1 мкг/кг/мин. Вторая группа получала стандартную терапию.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с ОДСН на фоне 24-часовой инфузии левосимендана значимо увеличилась скорость клубочковой фильтрации с 65,4 [45,2; 99,2] мл/мин/1,73м2 исходно до 79,0 [66,3; 93,1] мл/мин/1,73м2 при выписке (p=0,011), значительно снизился уровень креатинина c 103,4 [79,7; 137,3] мкмоль/л исходно до 89,4 [78,4, 100,6] мкмоль/л при выписке (p=0,009), а также значительно снизился уровень мочевины и одновременно улучшился почечный кровоток, в то время как в группе стандартной терапии не выявлено клинически значимого изменения исследуемых параметров.</p></sec><sec><title>Заключение</title><p>Заключение.
