Eptifibatide achieves high local concentration via direct intracoronary (I/V) injection as it promotes clot disaggregation, but it remains unclear if it is of superior benefit than the routine intravenous (I/V) administration. Aim. The current study aimed to examine the safety and efficacy of I/C vs I/V bolus regimen dose of eptifibatide during primary percutaneous coronary intervention (PPCI).</p></sec><sec><title>Material and methods</title><p>Material and methods. Prospective, controlled, randomized study enrolled 100 patients with acute anterior ST-segment elevation myocardial infarction (STEMI) eligible for PPCI equally divided into 2 groups (group A received bolus I/C eptifibatide and group B received it I/V) followed by 12-hour continuous I/V infusion. Features related to of myocardial salvage in the form of TIMI flow grade 3, myocardial blush grade 3, ST segment resolution and left ventricular systolic function were evaluated with short-term follow up for 1 month.</p></sec><sec><title>Results</title><p>Results. Mean age of the study population was 50.95±8.45years, there was statistically insignificant difference between both groups regarding baseline characteristics in age (p=0.062), gender (p=0.488), coronary artery disease risk factors (p&gt;0.05), time from onset of pain to admission (p=0.86) or door to balloon (p=0.12). Group A achieved statistically significant better myocardial blush grade 3 (42% vs 10%, p=0.005), ejection faction 30 days after PPCI (46.11±7.81% vs 40.88±6.26%, p=0.005) but statistically insignificant TIMI flow grade 3 (p=0.29) and ST resolution (p=0.34). Incidence of complications in the hospital and 30 days after discharge was statistically insignificant (p&gt;0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. Both regimens were safe and effective in STEMI patients undergoing PPCI and regimen of I/C bolus eptifibatide achieved better myocardial blush grade and systolic function. </p></sec></abstract><trans-abstract xml:lang="ru"><p>Интракоронарное (и/к) введение эптифибатида позволяет достичь высокой местной концентрации препарата, что способствует дезагрегации тромба, но остается неясным, есть ли у такого пути введения преимущества по сравнению с традиционным внутривенным (в/в) введением.</p><sec><title>Цель</title><p>Цель. Изучение эффективности и безопасности и/к болюсного введения эптифибатида по сравнению с в/в болюсным введением во время проведения первичного чрескожного коронарного вмешательства (ПЧКВ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проспективное контролируемое рандомизированное исследование включало 100 пациентов с острым передним инфарктом миокарда с подъемом сегмента ST (ИМпST), подходящих для проведения ПЧКВ и разделенных в соотношении 1:1 на две группы: в группе А болюсная доза эптифибатида вводилась и/к, в группе Б – в/в, затем пациентам обеих групп эптифибатид вводился в виде 12-часовой в/в инфузии. Такие признаки восстановления миокарда, как коронарный кровоток 3 степени по классификации TIMI, миокардиальное свечение 3 степени, регрессия сегмента ST, а также систолическая функция левого желудочка, оценивались спустя 1 мес наблюдения.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст участников исследования был 50,95±8,45 лет. При включении в исследование различия между двумя группами по параметрам: возраст, пол, наличие факторов риска ишемической болезни сердца, время от появления боли за грудиной до поступления в стационар, время от двери приемного отделения до введения катетера были статистически незначимыми, p=0,062, p=0,488, p&gt;0,05, p=0,86, p=0,12, соответственно. В группе А по сравнению с группой Б в значимо большем количестве случаев было достигнуто свечение миокарда 3 степени (42% и 10%; p=0,005), отмечены более высокие показатели фракции выброса левого желудочка спустя 30 дней после ПЧКВ (46,11±7,81% и 40,88±6,26%, p=0,005); при этом различия в частоте достижения 3 степени кровотока по TIMI и снижения сегмента ST были незначимыми: p=0,29 и p=0,34, соответственно. Различия в частоте осложнений в стационаре и спустя 30 дней после выписки были статистически незначимыми (p&gt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение.
