To determine the role of adherence to the basic drug treatment of heart failure (HF) in prevention of late major adverse events (MAEs) after isolated coronary artery bypass grafting (CABG) in patients with stable coronary artery disease (CAD) and left ventricular (LV) dysfunction at three-year follow-up.</p></sec><sec><title>Material and methods</title><p>Material and methods. A prospective non-controlled single-center study included 125 consecutive patients with stable CAD and LV EF&lt;50% (62±8 years; 114 [91.2%] males), after isolated CABG. At three-year follow-up MAЕs occurred in 40 (32.0%) patients. The data on pharmacotherapy at followup were obtained in 124 patients: 85 (68.6%) patients without MAEs and 39 (31.4%) patients with MAEs.</p></sec><sec><title>Results</title><p>Results. The enrolled sample of patients was characterized by high discharge prescription rate of renin-angiotensin system (RAS; 86.3%) blockers (angiotensin-converting enzyme inhibitors or angiotensin-II receptors blockers), beta-blockers (BBs; 97.6%) and mineralocorticoid receptors antagonists (MRAs; 79.0%), being comparable in MAEs and non-MAEs groups. The total coverage of basic HF pharmacotherapy (the combination of RAS blockers, BBs and MRAs) at discharge was 66.1%. At follow-up, about one third of patients in both groups withheld previously prescribed triple HF therapy. The MAEs were associated with more frequent withhold of previously prescribed RAS blockers, as opposed to patients without MAEs (20.5% and 7.1%, respectively; р=0.009). The majority of patients in both groups continued BBs therapy at follow-up (95.0% and 92.9%, respectively; p=0.187). Additionally, we observed the decline of MRAs intake frequency at follow-up (to 43.6% and 49.4%, respectively; p=0.547).</p></sec><sec><title>Conclusion</title><p>Conclusion. During 3-year follow-up after isolated CABG, about one third of patients with stable CAD and baseline LVEF&lt;50% interrupted triple basic HF therapy (including RAS blockers, BBs and MRAs), mainly due to decrease of RAS blockers and MRAs usage. MAEs in patients with stable CAD and baseline LVEF&lt;50% after CABG were associated with suboptimal use and more frequent interruption of RAS blockers.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель. Изучить роль приверженности к базисной терапии сердечной недостаточности (СН) у пациентов со стабильной ишемической болезнью сердца (ИБС) и дисфункцией левого желудочка (ЛЖ) в течение 3 лет после изолированного аортокоронарного шунтирования (АКШ) для профилактики поздних больших неблагоприятных событий (БНС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В проспективном неконтролируемом одноцентровом исследовании проанализировали данные, полученные при клиническом и инструментальном обследовании 125 пациентов со стабильной ИБС и фракцией выброса (ФВ) ЛЖ&lt;50% (возраст 62±8 лет; 114 [91,2%] мужчин), у которых было проведено изолированное АКШ. При 3-летнем наблюдении сердечно-сосудистые БНС произошли у 40 (32,0%) пациентов. Данные о базисной фармакотерапии СН в динамике наблюдения были получены у 124 пациентов: 85 (68,6%) пациентов без БНС, и 39 (31,4%) – с БНС.</p></sec><sec><title>Результаты</title><p>Результаты. Проанализированная когорта пациентов характеризовалась высокой частотой назначения блокаторов ренин-ангиотензиновой системы (РАС; 86,3%): ингибиторов ангиотензин превращающего фермента, либо блокаторов рецепторов ангиотензина ІІ; бета-адреноблокаторов (ББ; 97,6%) и антагонистов минералокортикоидных рецепторов (АМКР; 79,0%) которая была сопоставима в группах с и без БНС. Охват назначением базисной терапии СН, включающей одновременный прием блокатора РАС, ББ и АМКР, в общей выборке включенных пациентов составил 66,1%. В динамике наблюдения в каждой из групп сравнения примерно треть пациентов прекратили назначенную ранее трехкомпонентную базисную терапию СН. Возникновение БНС чаще ассоциировалось с прекращением приема ранее назначенных блокаторов РАС по сравнению с пациентами без БНС (20,5% и 7,1%, соответственно; р=0,009). В обеих группах преобладали случаи продолжения применения ранее назначенных ББ (95,0% и 92,9%, соответственно; р=0,187). В то же время в динамике наблюдения выявили снижение частоты применения АМКР до 43,6% и 49,4%, соответственно (р=0,547).</p></sec><sec><title>Заключение</title><p>Заключение. В течение 3 лет после изолированного АКШ примерно треть пациентов со стабильной ИБС и ФВ ЛЖ&lt;50% прекратили назначенную ранее терапию комбинацией 3 нейрогуморальных модуляторов преимущественно за счет снижения частоты применения блокаторов РАС и АМКР.
