To study the main relationships of the white coat effect (WCE) levels in patients with arterial hypertension (AH) with bronchial asthma (BA) who treated with AH and BA drugs in routine clinical practice.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. We analyzed the prospective cohort study data of AH patients, some of them had BA without exacerbation. We have formed two groups of patients:1 - control group, patients with AH without BA, the second - with AH + BA. The study consisted of three visits (first visit, 6 months and 12 months visits) and data collection period (30.1±7.6 months of follow-up). The following procedures were performed at the first and 12 month visits: clinical blood pressure (BP) measurements (sitting and standing), 24-hour monitoring ambulatory (ABPM), spirometry, clinical and biochemical blood tests, BA control questionnaires (ACQ) and quality of life (QL) questionnaire (GWBQ), at the second visit clinical BP measurement was performed and, if necessary, the drug dose was corrected.</p></sec><sec><title>Results</title><p>Results. The study included 125 patients, 28 men, 97 women. The first group of AH patients without BA included 85 people, the second (AH + BA) - 40. In AH patients without BA with ischemic heart disease, arterial revascularization, regular alcohol intake and smoking we identified the association with the lower WCE levels. In AH+BA patients with diabetes mellitus, gastrointestinal diseases, higher education was identified WCE decrease. In AH patients without asthma we found inverse relationships WCE levels with respiratory function parameters, the nighttime BP decrease, heart rate and the difference between standing and sitting BP levels, and correlations with the EchoCG variables (the left ventricular hypertrophy (LVH) indices), with age, AH duration and body mass index (BMI). In patients with AH + BA we found inverse correlations between WCE levels and some EchoCG variables, the difference between standing and sitting BP levels, and correlations with body weight, BMI.</p></sec><sec><title>Conclusion</title><p>Conclusion. Thereby, in AH patients without BA with ischemic heart disease, revascularization, regular alcohol intake, smoking we identified the association with the lower WCE levels. This patients WCE indices had inverse correlations with height, respiratory function parameters, the BP nighttime decrease, the difference between standing and sitting BP levels and direct relationships with EchoCG variables of LVH, age, AH duration and BMI. In AH + BA patients with diabetes, gastrointestinal diseases, higher education we found relationships with lesser WCE manifestation. WCE levels in this group had inverse correlations with some EchoCG indicators, the standing and sitting BP difference and direct relations with weight and BMI.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель. Изучить основные взаимосвязи показателей, характеризующих эффект «белого халата» (ЭБХ) у больных с артериальной гипертензией (АГ) в сочетании с бронхиальной астмой (БА), которые получали препараты для лечения АГ и БА в условиях рутинной клинической практики.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Были проанализированы данные проспективного когортного исследования больных с АГ, часть из которых имели БА вне обострения. Были сформированы две группы больных: в первую, контрольную группу были включены пациенты с АГ без БА, во вторую -с АГ+БА. Исследование состояло из трех визитов (исходный визит, через 6 и через 12 мес) и периода сбора данных об исходах (30,1±7,6 мес наблюдения). На 1 и 3 визитах проводили клинические измерения АД (сидя и стоя), суточное мониторирование (СМАД), спирометрию, клинический и биохимический анализы крови, анкетирование опросниками, оценивающими контроль над БА (ACQ) и КЖ больных (опросник GWBQ), на 2 визите выполнялось клиническое измерение АД и при необходимости - коррекция доз принимаемых препаратов.</p></sec><sec><title>Результаты</title><p>Результаты. В исследование было включено 125 больных, 28 мужчин, 97 женщин. В первую группу пациентов было включено 85 человек, во вторую - 40. У больных 1 группы наличие таких признаков, как ишемическая болезнь сердца, реваскуляризации артерий в анамнезе, регулярный прием алкоголя, курение было связано с меньшей выраженностью ЭБХ. У пациентов 2 группы наличие сахарного диабета, заболеваний желудочно-кишечного тракта, высшего образования сопровождалось меньшим проявлением ЭБХ по сравнению с остальными. В исследуемых группах были выявлены корреляции между выраженностью ЭБХ и следующими параметрическими показателями: в 1 группе больных были обнаружены обратные взаимосвязи между уровнями ЭБХ и показателями функции внешнего дыхания, степенью ночного снижения АД и ЧСС, разностью между АД стоя и АД сидя, и прямые корреляции с переменными эхокардиографии, характеризующими выраженность гипертрофии левого желудочка, с возрастом, продолжительностью АГ и индексом массы тела. У пациентов 2 группы были выявлены обратные корреляции между выраженность ЭБХ и показателями эхокардиографии, отражающими функциональное состояние левого желудочка, разностью между уровнем АД стоя и АД сидя, а также прямые корреляционные связи с массой тела, индексом массы тела.</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, для больных с АГ без БА наличие ИБС, реваскуляризации в анамнезе, регулярный прием алкоголя, курение было связано с меньшим проявлением ЭБХ, а уровень ЭБХ зависел от роста, функции внешнего дыхания, степени ночного снижения АД, от разности между показателями АД стоя и сидя (обратные связи), гипертрофии левого желудочка, возраста, продолжительности АГ, индекса массы тела (прямые взаимосвязи). У пациентов из группы АГ+БА присутствие таких факторов, как СД, заболевания желудочно-кишечного тракта, высшее образование характеризовалось меньшей выраженностью ЭБХ.
