To perform a population analysis of Non-High Density Lipoprotein Cholesterol level (non-HDL-c) in Russian population and to evaluate its association with cardiovascular events.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The material consisted of results obtained from 11 regions of the ESSE-RF1 Study and from 4 regions of the ESSE-RF2 Study. Study protocols were identical. The studies were performed in 2012-2014 and 2017, respectively. Endpoints were assessed in 19041 people aged 35-64 years. The median follow-up was 6.5 years in ESSE RF (1) and 3.8 years in ESSE RF(2). Analysis was performed for three lipid variables: total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and non-HDLC in two samples: the general population sample and the same sample without individuals with coronary heart disease (CHD), myocardial infarction (MI) and/or stroke history and not taking statins (the population sample of "without a history of cardiovascular diseases [CVD]". The analysis of nonlinear associations was performed using the generalized additive Cox model. The combined cardiovascular endpoint was represented by cardiovascular death and nonfatal MI and stroke. Traditional and laboratory FRs, socio-demographic parameters were analyzed. The significance level for all tested hypotheses was set to be 0.05.</p></sec><sec><title>Results</title><p>Results. The prevalence of elevated non-HDL-C level (&gt;3.7 mmol/l) was found to be 74.6%. No gender differences were found: there was 74.6% for men and 74.5% for women. Both mean values and prevalence of elevated non-HDL-C were increased with age in women, and its level was slightly decreased in men after 55 years old. Almost all analyzed RFs were significantly associated with elevated non-HDL-C in these two population samples. In both samples elevated total CH and elevated LDL-C were associated with all-cause mortality after correction for all RFs. On the contrary, the non-HDL-C was associated with CVD combined end pints. It has been shown that the risk of these end points increases uniformly with increase in levels of non HDL cholesterol, no nonlinear associations were found.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of a population-based analysis of non-HDL-C performed in the Russian population for the first time confirmed that elevated non-HDL-C levels contribute significantly to determining the risk of cardiovascular events in the medium term. It can be assumed that the new risk scales (SCORE2 and SCORE OP) proposed by the European Society of Cardiology and the European Society of Preventive Cardiology, which include non-HDL C instead of TC, will allow adequate assessment of 10-year cardiovascular risk for Russians. However, continued monitoring of endpoints in order to obtain stable associations is required.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель. Изучить популяционные характеристики холестерина, не входящего в состав липопротеинов высокой плотности (ХС неЛВП) и оценить его ассоциации с сердечно-сосудистыми событиями.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Результаты получены в 11 регионах исследования ЭССЕ-РФ1 и в 4 регионах исследования ЭССЕ-РФ2. Протоколы исследований были идентичны. Исследования выполнялись в 2012-2014 и 2017 гг., соответственно. Выживаемость оценена у 19041 человек 35-64 лет, медиана возраста составила 52 (44; 58) лет. Медиана проспективного наблюдения составила 6,5 (5,84; 6,83) лет в ЭССЕ-РФ1 и 3,82 (3,65; 3,86) лет в ЭССЕ-РФ2. Анализ проводился для трех липидных показателей: общий холестерин (ОХС), холестерин липопротеинов низкой плотности (ХС ЛНП) и ХС неЛВП в двух выборках: в общей популяционной выборке и выборке «без анамнеза сердечно-сосудистых заболеваний (ССЗ)», включающей лиц без ишемической болезни сердца, инфаркта миокарда (ИМ) и/или мозгового инсульта в анамнезе и не принимающих статины. Анализ нелинейных ассоциаций проводился при помощи обобщенной аддитивной модели Кокса. Комбинированная конечная точка (ККТ) представлена смертью от ССЗ и нефатальными инфарктом миокарда и мозговым инсультом. Анализировали традиционные и лабораторные факторы риска (ФР), социально-демографические показатели. Уровень значимости для всех проверяемых гипотез принят равным 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. Были изучены половозрастные показатели ХС неЛВП в общей выборке. Выявлена высокая распространенность повышенного уровня ХС неЛВП (&gt;3,7 ммоль/л) - 74,6%. Гендерных различий найдено не было: 74,6% у мужчин и 74,5% у женщин. С возрастом как средние значения, так и распространенность повышенного уровня ХС неЛВП нарастали у женщин, тогда как у мужчин после 55 лет отмечалось их незначимое снижение. Практически все ФР, включенные в анализ, значимо ассоциировались с повышенным уровнем ХС неЛВП в анализируемых выборках. В обеих выборках, после коррекции на все ФР, были выявлены ассоциации повышенных уровней ОХС и ХС ЛНП со смертностью от всех причин. Напротив, уровень ХС неЛВП ассоциировался с ККТ. Показано, что риск возникновения ККТ равномерно нарастает при увеличении уровня ХС неЛВП, т.е. показано отсутствие нелинейности ассоциаций.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты популяционного анализа уровня ХС неЛВП, впервые проведенного у населения России, подтверждают, что повышенный уровень ХС неЛВП вносит значительный вклад в детерминацию риска сердечно-сосудистых событий в среднесрочной перспективе. Можно полагать, что новая шкала риска (SCORE2 и SCORE OP), предложенная Европейским обществом кардиологов и Европейским обществом профилактической кардиологии, в которой вместо ОХС включен ХС неЛВП, позволит адекватно оценивать 10-летний сердечно-сосудистый риск россиян.
