WCE is due to the patient’s anxious response to office blood pressure (BP) measurement and is characterized by a BP increase. In patients with HTN, WCE is detected with a high frequency. Pronounced WCE in hypertensive patients is associated with worsening the HTN course and the prognosis of all-cause and cardiovascular mortality. In addition, BP increase in these patients during a doctor’s visit may lead to additional prescription of antihypertensive drugs, increasing costs and side effects. To identify WCE, a large number of methods are used, which can be divided into following groups: I – data from clinical and daytime ambulatory blood pressure measurement, II – clinical and home BP assessment, III – only ambulatory BP measurement, IV – clinical and laboratory BP data, and V – only clinical BP measurement. There are following WCE in patients with HTN: female sex, age, never smoking, as well as anxiety, introversion, etc. WCE is due to the patient’s anxiety state during office BP measurement and a conditioned reflex BP increase associated both with the physician presence and medical background. Severe WCE in some groups of patients with HTN may be associated with a worse prognosis compared with other hypertensive patients as follows: in individuals with a significant difference between clinical and ambulatory blood pressure levels or between the first and third BP office data, with a combination of resistant HTN and WCE, with WCE increasing from visit to visit, etc.</p></abstract><trans-abstract xml:lang="ru"><p>Статья посвящена различным аспектам "эффекта белого халата" (ЭБХ) у пациентов с артериальной гипертензией (АГ). Феномен ЭБХ является отражением тревожной реакции пациента на измерение артериального давления (АД) врачом в клинике и характеризуется прессорным ответом АД. У пациентов с АГ ЭБХ выявляется с высокой частотой. Выраженный ЭБХ у больных с АГ связан с ухудшением течения АГ и прогноза общей и сердечно-сосудистой смертности. Кроме того, повышение АД у этих больных на визите у врача, может стать причиной дополнительного назначения антигипертензивных препаратов, увеличения затрат и количества побочных эффектов. Для выявления ЭБХ используется большое количество методик, которые можно разделить на основные группы: первая — использует данные клинических и дневных амбулаторных измерений АД, вторая — клинических и домашних, третья — амбулаторных, четвертая — клинических и лабораторных, пятая — только клинических измерений АД. Первая группа методик наиболее широко используется в клинических исследованиях. Предикторами наличия ЭБХ у пациентов с АГ может стать женский пол, возраст, отсутствие курения в анамнезе, а также показатели тревожности, интроверсии и др. Механизм ЭБХ связан с тревожным состоянием пациента во время измерения АД врачом и условно-рефлекторным повышением АД, связанным не только с присутствием врача, но и с медицинской обстановкой. Выраженный ЭБХ у некоторых групп пациентов с АГ может быть связан с худшим прогнозом по сравнению с другими субъектами с АГ: у лиц со значительной разницей между клиническими и амбулаторными уровнями АД или между первым и третьим показателями АД при стандартном измерении врачом в клинике, при сочетании резистентной АГ и ЭБХ, при усиливающейся от визита к визиту ЭБХ и др.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>артериальная гипертензия</kwd><kwd>эффект белого халата</kwd><kwd>суточное мониторирование артериального давления</kwd></kwd-group><kwd-group xml:lang="en"><kwd>hypertension</kwd><kwd>white coat effect</kwd><kwd>24-hour blood pressure monitoring</kwd></kwd-group><funding-group xml:lang="ru"><funding-statement>При поддержке ФГБУ НМИЦ ТПМ</funding-statement></funding-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Ерина А.
