Представлен анализ лечения ривароксабаном в заранее определенной подгруппе больных инфарктом миокарда (ИМ) с подъемом сегмента ST, антикоагулянтная терапия которых вызывает особый интерес.</p></sec><sec><title>Введение</title><p>Введение. Ранее в исследовании ATLAS ACS-2-TIMI-51 (Anti-Xa Therapy to Lower Cardiovascular Eventsin Addition to Standard Therapy in Subjects with Acute Coronary Syndrome – Thrombolysis In Myocardial Infarction-51) было показано, что ривароксабан снижал частоту сердечно-сосудистых событий у больных с любыми формами острого коронарного синдрома.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование ATLAS ACS-2-TIMI-51 были включены 7817 больных ИМ с подъемом сегмента ST. После стабилизации состояния (через 1-7 дней) они были рандомизированы в группы лечения ривароксабаном в дозе 2,5 мг 2 р/д, ривароксабаном в дозе 5 мг 2 р/д, или плацебо. Результаты представлены как кривые Каплана-Мейера для частоты исходов в течение 2 лет, проводился анализ в соответствии с назначенным лечением («intention to treat» – ITT) и модифицированный анализ в соответствии с назначенным лечением (m-ITT).</p></sec><sec><title>Результаты</title><p>Результаты. У больных ИМ с подъемом сегмента ST ривароксабан снизил в сравнении с плацебо частоту первичной конечной точки, включавшей смерть от сердечно-сосудистых причин, ИМ и инсульт (ITT: 8,4% в сравнении с 10,6%, отношение рисков [ОР]: 0,81, 95% доверительный интервал [ДИ] от 0,67 до 0,97, р=0,019; m-IIT: 8,3% в сравнении с 9,7%, ОР: 0,85, 95% ДИ: 0,70-1,03, р=0,09). Это снижение выявлялось через 30 дней (ITT и m-ITT: 1,7% в сравнении с 2,3%, р=0,042) и было очевидным при анализе исходов, произошедших в период, когда пациенты получали фоновую двойную антиагрегантную терапию (ITT: 7,9% в сравнении с 11,9%, р=0,010; m-ITT: 7,7% в сравнении с 10,1%, р=0,061). Относительно индивидуальных доз: доза ривароксабана 2,5 мг снижала сердечно-сосудистую смертность (ITT: 2,5% в сравнении с 4,2%, р=0,006; m-ITT: 2,2% в сравнении с 3,9%, р=0,006), но такого эффекта не было получено при лечении ривароксабаном в дозе 5 мг. Ривароксабан в сравнении с плацебо повышал частоту не связанных с коронарным шунтированием больших кровотечений (в соответствии с определением TIMI – Thrombolysis in Myocardial Infarction) (2,2% против 0,6%, р&lt;0,001) и внутричерепных кровоизлияний (0,6% против 0,1%, р=0,015) без значительного увеличения частоты смертельных кровотечений (0,2% против 0,1%, р=0,51).</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов, недавно перенесших ИМ с подъемом сегмента ST, ривароксабан снижал частоту сердечно-сосудистых событий. Это преимущество проявилось рано и сохранялось на протяжении периода, когда больные получали фоновую двойную антиагрегантную терапию.
