Inflammasomes are cytoplasmic high-molecular protein complexes, which activation leads to the development of inflammation through the release of proinflammatory cytokines. The most studied types of inflammasomes are NLRP3 inflammasomes, which contribute to the development of systemic and local inflammation, including in the myocardium. The NLRP3 inflammasome comprises the receptor/sensor protein NLRP3 (nucleotide-binding leucine-rich repeat receptor pyrin domain-containing-3), the adaptor protein ASC (apoptosis-associated speck-like protein containing CARD), and the effector protein caspase-1. The NLRP3 receptor protein, as a structural component of the NLRP3 inflammasome, functions as an intracellular pattern-recognition receptor that detects pathogen-associated molecular patterns (PAMPs) and danger-associated molecular patterns (DAMPs, molecules from damaged cells). Activation of this receptor triggers the assembly of the NLRP3 inflammasome, leading to the release of interleukin-1β and interleukin-18. The article presents data on studies of inflammasome profile biomarkers and the possibility of their use in order to improve diagnostics, prognosis and personalisation of treatment of heart failure. The accumulated data of clinical and experimental studies convincingly indicate the necessity for further comprehensive analysis of inflammasome profile proteins in patients with heart failure.</p></abstract><trans-abstract xml:lang="ru"><p>Цель обзора — провести анализ публикаций, посвященных исследованиям белков инфламмасомного профиля у больных с хронической сердечной недостаточностью (ХСН) в качестве потенциальных диагностических маркеров. Инфламмасомы представляют собой цитоплазматические высокомолекулярные белковые комплексы, активация которых приводит к развитию воспаления посредством высвобождаемых провоспалительных цитокинов. К наиболее изученным типам инфламмасом относят NLRP3-инфламмасомы, которые вовлечены в развитие системного и локального воспаления, в том числе в миокарде. В состав NLRP3-инфламмасомы входят рецепторный/сенсорный белок NLRP3 (nucleotide-binding leucine-rich repeat receptor pyrin domain-containing-3), адапторный белок ASC (apoptosis-associated speck-like protein containing CARD) и эффекторный белок — фермент каспаза-1. Рецепторный белок NLRP3, являясь структурным компонентом инфламмасомы NLRP3, выполняет функцию внутриклеточного паттерн-распознающего рецептора, который распознает патоген-ассоциированные молекулярные паттерны (pathogen-associated molecular patterns, PAMPs) и молекулярные сигналы опасности (danger-associated molecular patterns, DAMPs, молекулы поврежденных клеток). Активация данного рецептора приводит к сборке инфламмасом NLRP3, с последующим высвобождением интерлейкин-1β и интерлейкин-18. В обзоре приводятся данные о строении и классификации инфламмасомы NLRP3, рассмотрены пути ее активации, в том числе при ХСН, обсуждается роль инфламмасомы NLRP3 в патогенезе ХСН в сочетании с фибрилляцией предсердий, артериальной гипертонией, сахарным диабетом 2 типа и ожирением. Особое внимание в обзоре уделено анализу результатов исследований биомаркеров инфламмасомного профиля, возможности их количественного определения и применения с целью улучшения диагностики, прогнозирования и персонализации лечения ХСН.
