Тo conduct a preclinical trial of the effect of beetroot powder (Beta-NO-BOOST), as an exogenous source of nitrate and nitrite ions (NOx), on blood pressure (BP) and arterial stiffness in patients with arterial hypertension (AH).</p></sec><sec><title>Material and methods</title><p>Material and methods. The pilot preclinical study included 22 male volunteers (49±10 years) with AH who completed the protocol. Participants took Beta-NOBOOST beetroot powder, developed for the National Research Center for Therapeutic and Mechanics, at a dose of 20 g/day, equivalent to 650-700 mg NOx, orally for a week in addition to prescribed therapy. Blood pressure was monitored by 24-hour BP monitoring and volumetric sphygmography, which included measuring blood pressure in all four limbs and calculating the cardio-ankle vascular index (CAVI) and ankle-brachial index before and after beetroot powder intake. Blood was collected to analyze the total concentration of nitrate and nitrite ions (NOx, μM) using a colorimetric method based on the Griess reaction with single-step reduction of nitrate to nitrite using a vanadium (III) chloride solution in 1N hydrochloric acid.</p></sec><sec><title>Results</title><p>Results. Serum NOx concentrations increased by an average of 2.7 times after completing the Beta-NO-BOOST beetroot powder regimen, confirming adherence to the study protocol. No significant difference in BP levels was observed before and after Beta-NO-BOOST, but a significant decrease of 0.3 units in the arterial stiffness index (CAVI) was noted. Furthermore, after a week of Beta-NO-BOOST regimen, negative correlations were found between NOx concentrations and blood pressure parameters, with a high r coefficient reaching -0.66.</p></sec><sec><title>Conclusion</title><p>Conclusion. This preclinical trial demonstrated that increased NOx concentrations in the blood were associated with decreased arterial stiffness. Based on the significant negative correlations between serum NOx levels and blood pressure, it is demonstrated that the NOx in beetroot powder contributes to lowering blood pressure. The results of the preclinical trial support the feasibility of a randomized crossover controlled clinical trial of the developed beetroot powder, Beta-NO-BOOST, as a therapeutic nutritional supplement for patients with AH.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Цель</title><p>Цель. Провести доклиническое испытание влияния порошка корнеплода свеклы (Beta-NO-БУСТ), как экзогенного источника нитрат- и нитрит-ионов (NOx), на уровень артериального давления (АД) и показателей артериальной жесткости у пациентов с артериальной гипертонией (АГ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В пилотное доклиническое исследование было включено 22 добровольца мужского пола (49±10 лет) с АГ. Участники принимали свекольный порошок «Beta-NO-БУСТ», разработанный по заказу ФГБУ «НИМЦ ТПМ» в дозе 20 г/день, эквивалентной 650-700 мг NOx перорально в течение недели дополнительно к терапии, если таковая была назначена. У участников проводили контроль АД путем СМАД, а также объемную сфигмографию, включающую измерение АД на 4-х конечностях с расчетом индекса CAVI (cardio-ankle vascular index) или сердечно-лодыжечного сосудистого индекса и лодыжечно-плечевого индекса до и после приема порошка свеклы. Проводилось взятие крови для анализа суммарной концентрации нитрат- и нитрит-ионов (NOx, мкМ) колориметрическим методом, основанным на реакции Грисса с одноэтапным восстановлением ионов нитрата в нитрит с помощью раствора хлорида ванадия III в 1Н соляной кислоте.</p></sec><sec><title>Результаты</title><p>Результаты. По завершении приема свекольного порошка «Beta-NO-БУСТ», концентрация NOx в сыворотке крови участников возросла в среднем в 2,7 раз, что подтверждает выполнение ими условий протокола исследования. Значимых различий в уровне АД до и после приема Beta-NO-БУСТ выявлено не было, но отмечалось значимое снижение на 0,3 единицы показателя артериальной жесткости – CAVI. Также, после недельного курса Beta-NO-БУСТ были выявлены отрицательные корреляции между концентрацией NOx и параметрами АД с высоким коэффициентом r, достигавшим величины -0,66.</p></sec><sec><title>Заключение</title><p>Заключение. В ходе доклинического испытания было показано, что при повышении концентрации NOx в крови наблюдалось снижение показателя артериальной жесткости. На основании выраженных отрицательных корреляций между уровнями NOx в сыворотке и уровнем АД, убедительно показано, что именно NOx в составе свекольного порошка способствует снижению АД.
