According to recent guidelines, oral dabigatran etexilate is indicated for stroke and systemic embolism prevention in patients with atrial fibrillation (AF). Aim. Based on the RE-LY study to evaluate the cost-effectiveness of dabigatran etexilate versus warfarin prescribed in “real-world” settings from a Russian payer perspective. Material and methods. Markov model simulated AF patients at moderate to high risk of stroke while tracking clinical events and resulting functional disability. Acute event costs and resulting long-term follow-up costs incurred by disabled stroke survivors were calculated using general tariff agreement of Russian obligatory health insurance system and official national statistics. Clinical events, summarized as events per 100 patient-years, expected life years, total costs, and incremental cost effectiveness ratios (ICER) were calculated. Results. Over a lifetime, dabigatran etexilate treated patients experienced fewer intracranial haemorrhages and fewer ischaemic strokes. ICER of dabigatran etexilate was 461,602 roubles per one additional life year versus “real-world” warfarin. Conclusion. This study demonstrates that dabigatran etexilate is a cost-effective alternative to current care for the prevention of stroke and systemic embolism among Russian patients with AF .

В соответствии с современными рекомендациями, дабигатрана этексилат может назначаться с целью профилактики инсульта и системных тромбоэмболий у пациентов с фибрилляцией предсердий (ФП). Цель. Оценить фармакоэкономическую эффективность применения дабигатрана этексилата в сравнении с варфарином в профилактике сердечно-сосудистых осложнений у пациентов с ФП в условиях системы обязательного медицинского страхования (ОМС) в России. Материал и методы. В ходе исследования была построена модель Маркова, в рамках которой моделировали когорту пациентов с ФП со средним и высоким риском развития инсульта, при этом принимали во внимание сердечно-сосудистые осложнения и инвалидизацию. Затраты на лечение осложнений и ведение пациентов с последовавшей после инсульта инвалидностью были подсчитаны с использованием тарифов системы ОМС и с учетом официальных данных статистики. Были рассчитаны сердечно-сосудистые осложнения, ожидаемая продолжительность жизни, общие затраты и показатели клинико-экономической эффективности в расчете на 100 пациенто-лет. Результаты. В периоде наблюдения, соответствующего продолжительности жизни моделируемых пациентов, среди когорты, «получавшей» дабигатрана этексилат , количество ишемических и геморрагических инсультов было меньшим. Рассчитанные затраты на 1 дополнительный год жизни при применении дабигатрана составили 461 602 руб. Заключение. Показано, что дабигатрана этексилат может служить эффективной в фармакоэкономическом аспекте альтернативой варфарину у российских пациентов с ФП.
