Aim. To evaluate the changes of respiratory function in patients with rheumatic heart disease (RHD) complicated with pulmonary hypertension (PH) by the spirographic investigation. Materials and methods. Patients (n=56; 4 men and 52 women; aged 63,25±7,93) with rheumatic combined mitral and aortic valvular disease were examined. Medical history analysis, physical examination, general clinical investigation including ECG, chest X-ray, 2D-echocardiography and spirography were performed.  Results. 36% of the patients had normal pulmonary arterial systolic pressure (PASP) (<30 mm Hg), 48% had PH of degree I and 16% - PH of degree II. Restrictive changes of spirogram were found in 27% of patients: in 30% of patients with PH of degree I, in 44% of patients with PH of degree II and in 5% of patients with no PH. Bronchoobstruction was detected in no one patient. Pulmonary restriction significantly (p<0,05) correlated with PH, but no significant difference was found between groups with PH of I and II degree. No any association between PASP, spirogram changes and type of valvular lesion was revealed. Conclusion. Patients with RHD complicated with PH have pulmonary restrictive changes according to spirographic investigation.

Цель. Оценить изменения показателей функции внешнего дыхания (ФВД) у больных ревматической болезнью сердца (РБС), осложненной легочной гипертензией (ЛГ), с помощью спирографии. Материал и методы. Обследованы 56 больных с ревматическими сочетанными митральными и комбинированными митрально-аортальными пороками (4 мужчин, 52 женщины; возраст 63,25±7,93 года). У всех пациентов проведены сбор анамнеза, физикальное обследование, общеклинические исследования, включая ЭКГ и рентгенографию органов грудной клетки, 2Д-эхокардиография и исследование ФВД. Результаты. У 36% больных систолическое давление в легочной артерии (СДЛА) не превышало 30 мм рт.ст. (норма), у 48% степень повышения СДЛА соответствовала ЛГ 1 степени, у 16% — ЛГ 2 степени. Признаки рестриктивных изменений ФВД выявлены у 27% пациентов: у 30% больных с ЛГ 1 степени, у 44% - с ЛГ 2 степени и у 5% больных с нормальным показателем СДЛА. Ни в одном случае не зафиксировано бронхообструкции. Легочная рестрикция достоверно (р<0,05) ассоциировалась с наличием ЛГ, однако статистически значимых различий между группами пациентов с ЛГ 1 и 2 степени выявлено не было. Также не выявлено взаимосвязи уровня СДЛА и показателей спирограммы с типом клапанного поражения.  Заключение. У пациентов с РБС, осложненной ЛГ, имеет место достоверное изменение показателей ФВД по рестриктивному типу.
