Angiotensin converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARB) slow down progression of cardiovascular diseases and reduce risk of mortality and life threatening complications. What it is better to prescribe for patient in a concrete clinical case – ACE inhibitors or ARB? Authors compare these drug classes (mechanism of action, indications, evidense base of clinical trails, treatment costs and safety). The place of ACE inhibitors and ARB in modern therapy of cardiovascular diseases is defined. Results of the recent trails (ONTARGET, TRANCEND, PRoFESS, I-PRESERVE) are discussed.

Ингибиторы ангиотензинпревращающего фермента (иАПФ) и антагонисты рецепторов к ангиотензину (АРА) замедляют прогрессирование патологических изменений сердечно-сосудистой системы (ССС) и снижают риск летальности и угрожающих жизни осложнений при всех клинических проявлениях сердечно-сосудистого континуума. Поэтому очень часто возникает проблема выбора: что лучше применить в конкретной клинической ситуации - иАПФ или АРА? Авторы, сравнивая эти два класса лекарственных средств по целому ряду показателей (особенности механизма действия, зарегистрированные показания для каждой из групп препаратов, накопленная настоящему времени доказательная база клинических исследований, стоимость и переносимость лечения), четко определяют место иАПФ и АРА в современной терапии сердечно-сосудистых заболеваний. Приводятся и анализируются результаты последних исследований, таких как ONTARGET, TRANCEND, PRoFESS, I-PRESERVE, значительно проясняющих суть обсуждаемой проблемы.
