Целью исследования была оценка связи уровня привычного потребления калия с частотой сердечно-сосудистых заболеваний (ССЗ). Введение. Cвязь между привычным потреблением калия и частотой сосудистых заболеваний оценивали в ряде проспективных когортных исследований, однако результаты этих исследований не были полностью однородными. Материал и методы. Проведен систематический поиск проспективных исследований, опубликованных с 1966 года по декабрь 2009 на разных языках без ограничений. Критериям включения соответствовали проспективные исследования, проведенные в популяции взрослых, с оценкой исходного потребления калия, оценкой сосудистых событий в исходе и наблюдением не менее 4-х лет. В каждом из исследований были выбраны показатели относительного риска (ОР) с 95% доверительными интервалами (ДИ), которые затем были объединены с применением модели случайных эффектов, с «весами», обратно пропорциональными оценкам их дисперсий. Выполнялся анализ гетерогенности (неоднородности), систематической ошибки, связанной с предпочтительной публикацией положительных результатов исследования, анализ в подгруппах и мета-регрессионный анализ. Результаты. Было выявлено одиннадцать исследований, охватывающих 15 когорт выборных совокупностей, включивших всего 247 510 участников (мужчин и женщин), которых наблюдали от 5 до 19 лет , у которых было зарегистрировано 7 066 инсультов, 3 058 событий, относящихся к проявлениям ишемической болезни сердца (ИБС), и 2 497 событий, относящихся к ССЗ в целом. Потребление калия оценивали в этих исследованиях на основании сообщений участников о питании в течение предшествующих 24 ч (n=2), опросников о частоте приема пищевых продуктов (n=6) или суточной экскреции калия с мочой (n=3). При объединенном анализе увеличение потребления калия на 1,64 г (42 ммоль) в день было ассоциировано со снижением риска инсульта на 21% (ОР 0,79; 95%ДИ 0,68 до 0,90; р=0,0007) и тенденцией к более низкому риску ИБС и ССЗ в целом, которая приобретала статистическую значимость после основанного на анализе чувствительности исключения одной когорты (ОР 0,93; 95%ДИ 0,87 до 0,99; р=0,03; и ОР 0,74; 95%ДИ 0,60 до 0,91; р=0,0037, соответственно). Заключение. Более значительное потребление калия с пищей ассоциировано с более низкой частотой инсульта, а также может снижать риск ИБС и ССЗ в целом.
