Aim. To compare the antiplatelet efficacy of the original and generic enterosoluble tableted acetylsalicylic acid (ASA) in patients with high cardiovascular risk.

Material and мethods

Material and мethods. Patients (n=30) with high cardiovascular risk and indications for the ASA therapy were included in the study. Antiplatelet agents were withdrawn 3 weeks before study start (wash-out period). Study drugs were distributed at the start visit according to the randomization scheme. The first study drug course was 3 weeks, then next wash-out period lasting 3 weeks, and at last the second study drug course during 3 weeks occurred. Blood samples were taken; blood pressure and heart rate were recorded before and after therapies. ASA antiplatelet effect was evaluated by spontaneous and ADP-induced platelet aggregation assessment. Platelet aggregation was determined by the change turbidimetric translucent ability of the blood sample during the formation of aggregates after 2 minutes of exposure. ADP solution of three concentrations (0.5, 1 and 2 µMol) was used as an inductor of aggregation.

Results

Results. Significant differences between the compared drugs in effect on platelet aggregation, blood pressure and heart rate were not found. Signs of platelet desaggregation after 3 weeks of generic ASA therapy were found in 65, 34.5 and 31% of patients when aggregation was tested with ADP 0.5, 1 and 2 µMol respectively. Desaggregation was revealed in 65.4, 42.3 and 31% of patients treated with original ASA (p>0.05) in the same test and the same ADP inducing concentration.. Adverse events associated with study drugs were not registered.

Conclusion

Conclusion. ASA generic Trombopol ® 75 mg (Pharmaceutical Factory Polpharma SA, Poland) and ASA original Aspirin® Cardio (Bayer AG, Germany) are equivalent in antiplatelet effect and tolerability.

Цель

Цель. Сравнить дезагрегантную способность оригинального и воспроизведенного препаратов ацетилсалициловой кислоты (АСК), выпускаемых в виде таблеток в кишечнорастворимой оболочке, у больных высокого риска сердечно-сосудистых осложнений.

Материал и методы

Материал и методы. В исследование включены 30 пациентов с высоким риском сердечно-сосудистых осложнений и показаниями к применению препаратов АСК. До начала активного лечения больным, продолжающим обычную для них терапию, на 3 нед отменяли антиагреганты. На следующем визите пациенту выдавали один из исследуемых препаратов согласно схеме рандомизации. Изучаемые препараты принимались утром в одно и то же время. Контрольный визит проводился через 3 нед. Через 21 день после отмены первого изучаемого препарата проводили второй аналогичный курс терапии с другим изучаемым препаратом. На каждом визите перед приемом препарата отбирали пробы крови для определения АДФ-индуцированной агрегации тромбоцитов; измеряли артериальное давление (АД) и частоту сердечных сокращений (ЧСС). Дезагрегантную эффективность препаратов АСК оценивали по влиянию на спонтанную и АДФ-индуцированную агрегационную способность тромбоцитов. Агрегационную способность тромбоцитов определяли турбидиметрическим методом по изменению светопропускающей способности образца крови при образовании агрегатов после 2-х мин экспозиции. В качестве индуктора агрегации использовали раствор АДФ в трех концентрациях (0,5; 1 и 2 мкмоль).

Результаты

Результаты. Достоверных различий между сравниваемыми препаратами при ежедневном приеме по влиянию на агрегационную способность тромбоцитов, АД, ЧСС не выявили. Признаки дезагрегации после 3 нед терапии выявлены у 65, 34,5 и 31% больных при индукции АДФ в дозе 0,5, 1 и 2 мкмоль на фоне приема тестируемого и 65,4, 42,3 и 31% больных на фоне приема оригинального препаратов АСК (p>0,05). Нежелательных явлений, связанных с приемом изучаемых препаратов, не было.

Заключение

Заключение. Дженерический препарат АСК Тромбопол® 75 мг (Фармацевтический завод Польфарма АО, Польша) и оригинальный препарат Аспирин® Кардио (Bayer AG, Германия) эквивалентны по дезагрегантному действию и переносимости.
