В данном обзоре литературы представлены факторы риска инфекций ИСУ, микробиологические аспекты, патогенез, а также рассмотрены обоснование и доказательства использования чехлов, выделяющих антибиотики, у пациентов с повышенным риском инфекционных осложнений. В качестве профилактики используются чехлы ИСУ с антимикробной пропиткой которые были разработаны для надежного удержания устройств на месте, обеспечения стабильной среды и снижения риска инфекционных осложнений. Поскольку считается, что большинство инфекций происходит во время имплантации сердечных устройств, эти чехлы стали эффективным методом профилактики. Новые техно-</p><p>логии чехлов обещают снизить вероятность заражения ложа ИСУ. Доклинические и клинические данные подтверждают эффективность местной доставки антибиотиков через синтетические оболочки или оболочки ИСУ на основе внутриклеточного матрикса, элюирующие рифампицин/миноциклин и гентамицин, соответственно. Более того, оболочка внеклеточного матрикса обычно стимулирует ремоделирование тканей и ангиогенез, тем самым сводя к минимуму воспаление и способствуя клиренсу бактерий. Учитывая продолжающийся рост количества имплантации сердечных устройств и связанные с этим инфекционные осложнения, решающее значение имеют инновационные подходы к профилактике инфекций при их установке.</p></abstract><trans-abstract xml:lang="en"><p>Infections associated with cardiac implantable electronic devices (CIED) pose a serious risk to the patient’s health due to the high mortality rate of this type of complications. This literature review presents risk factors, microbiology and pathogenesis, as well as the rationale and evidence for the use of antibiotic-releasing envelopes in patients with an increased risk of infectious complications. As a preventive measure, envelopes for implantable cardiac devices with antimicrobial impregnation are used, which have been developed to reliably hold devices in place, provide a stable environment and reduce the risk of infectious complications. Since it is believed that most infections occur during cardiac devices implantation, these covers have become an effective method of prevention. New technologies of the envelopes promise to reduce the CIED pocket infection rate. Preclinical and clinical data confirm the effectiveness of local antibiotic delivery through synthetic shells or ICS shells based on intracellular matrix, eluting rifampicin/minocycline and gentamicin, respectively. Moreover, the shell of the extracellular matrix usually stimulates tissue remodeling and angiogenesis, thereby minimizing inflammation and promoting bacterial clearance. Given the continued growth in the implantation of cardiac devices and the associated infectious complications, innovative approaches to infection prevention during the implantation of arrhythmological devices are crucial.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>инфекции имплантируемых сердечных устройств</kwd><kwd>профилактика</kwd><kwd>чехол выделяющий антибиотик</kwd></kwd-group><kwd-group xml:lang="en"><kwd>infections of implanted cardiac devices</kwd><kwd>prevention</kwd><kwd>antibiotic releasing case</kwd></kwd-group></article-meta></front><body><sec><title>Введение</title><p>Инфекционные осложнения (ИО) являются наиболее неблагоприятными при использовании имплантируемых сердечных устройств (ИСУ), поскольку имеют высокий риск тяжелого течения и смертности. Более того, отмечается значительный рост зарегистрированных ИО на фоне непрерывного роста ИСУ в популяции [<xref ref-type="bibr" rid="cit1">1</xref>]. Это может быть связано с тем, что у пациентов с ИСУ имеются значимые факторы риска, такие как пожилой возраст и большое количество сопутствующих заболеваний [<xref ref-type="bibr" rid="cit1">1</xref>]. Таким образом, профилактика инфекции является важным аспектом применения ИСУ. Научно обоснованные профилактические подходы включают предоперационное введение антибиотиков, тщательную антисептическую обработку операционного поля при имплантации ИСУ и использование чехлов, выделяющих антибиотики (ЧВА) [<xref ref-type="bibr" rid="cit2">2</xref>]. Появляющиеся литературные данные подтверждают эффективность ЧВА, обеспечивающих высокие местные концентрации антибиотиков в ложе устройства с минимальным системным воздействием [<xref ref-type="bibr" rid="cit3">3</xref>]. В настоящее время в США имеется два коммерчески доступных чехла для имплантируемых сердечных устройств. На данный момент уже имеется доказательная база, обосновывающая использование ЧВА.</p></sec><sec><title>Инфекции имплантируемых сердечных устройств: заболеваемость, затраты здравоохранения и факторы риска</title><p>В настоящее время отсутствуют всеобъемлющие регистры инфекций имплантируемых сердечных устройств (ИИСУ), а имеющиеся данные в основном базируются на наблюдательных исследованиях и варьируют в зависимости от ИСУ и продолжительности наблюдения. Недавний анализ базы данных в США за 2016 г. показал, что суммарная вероятность ИО ИСУ в течение одного года составляет 1,18% при первичной имплантации и 2,37% — при замене устройства [<xref ref-type="bibr" rid="cit4">4</xref>]. В других исследованиях сообщалось, что частота ИИСУ достигает 7% после замены устройства [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Согласно рекомендациям, все виды ИО, связанных с ИСУ, делятся на локальные инфекции устройства и инфекционный эндокардит (ИЭ) ИСУ. И если первые ограничены ложем устройства и сопровождаются местными признаками воспаления с возможным развитием эрозий и несостоятельности раны, то ИЭ ИСУ сопровождаются распространением инфекции по электродам с поражением створок клапанов и эндокарда [<xref ref-type="bibr" rid="cit6">6</xref>]. Регламентированный перечень необходимого обследования, включающий проведение трансторакальной и чреспищеводной эхокардиографии, микробиологического исследования материала (крови, отделяемого ложа, извлеченного ИСУ), а в ряде случаев однофотонной эмиссионной компьютерной томографии миокарда с мечеными лейкоцитами или позитронно-эмиссионной томографии миокарда с фтордезоксиглюкозой, необходимость обширной антибиотикотерапии, в ряде случаев удаления имплантированной системы, имплантации временного кардиостимулятора и/или иного ИСУ формируют понимание колоссальных предполагаемых затрат здравоохранения, связанных с проблемой ИИСУ. Согласно отечественным клиническим рекомендациям по ИЭ и инфекциям внутрисердечных устройств, следует различать следующие сценарии развития ИО — локальные инфекции устройства и ИЭ, связанный с имплантируемым устройством. У пациентов с ИЭ и вегетациями на электродах рекомендовано рассмотреть возможность хирургического удаления электродов (до 10 мм эндоваскулярным, а более 20 мм кардиохирургическим способом). После удаления сердечного устройства всегда следует повторно рассмотреть показания к имплантации устройства [<xref ref-type="bibr" rid="cit6">6</xref>]. Данные исследований свидетельствуют о зависимости имеющихся экономических потерь от популяции пациентов, необходимости удаления имплантированной системы и/или госпитализаций, основной и сопутствующей патологии [7-9].</p></sec><sec><title>Микробиологические аспекты и патогенез инфекций имплантируемых сердечных устройств</title><p>Коагулазонегативные стафилококки (в первую очередь Staphylococcus epidermidis) и S. aureus составляют &gt;70% всех ИИСУ [<xref ref-type="bibr" rid="cit10">10</xref>]. Менее распространенные возбудители включают граммотрицательные кокки и грибы. Распространенность устойчивых к антибиотикам Staphylococcus spp. (особенно S. aureus) варьирует в разных исследованиях, зависит от географического положения исследуемой популяции, что следует учитывать при лечении ИО, связанных с ИСУ [<xref ref-type="bibr" rid="cit2">2</xref>]. Клиническая картина варьирует от локального инфекции ложа до бактериемии и эндокардита [<xref ref-type="bibr" rid="cit2">2</xref>]. Большинство ИИСУ, возникают из-за контаминации ложа кожной флорой во время первичной имплантации или последующих ревизионных вмешательств. Инфекция ложа ИСУ может затем распространяться по сосудистой системе, что приводит к инфицированию кровотока и клапанному эндокардиту. Иногда у пациентов может развиться гематогенное обсеменение электрода или ложа ИСУ из удаленного источника [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Первоначальное прикрепление микроорганизмов к устройству обусловлено как характеристиками организма, так и поверхности самого устройства. Микробы также могут соединяться с белками внеклеточного матрикса (ВКМ), синтезируемыми организмом пациента с ИСУ, которые покрывают поверхность устройства в ложе. По мере дополнительной контаминации колоний уже адсорбировавшихся на поверхности устройства, образуется биопленка из бактериальных слоев и межклеточных адгезинов, которая делает бактерии более устойчивыми к антибиотикам и иммунной защите, в частности, за счет ингибирования проникновения в локальную область [<xref ref-type="bibr" rid="cit2">2</xref>].</p></sec><sec><title>Профилактика инфекций имплантируемых сердечных устройств</title><p>Профилактика является предпочтительной стратегией снижения уровня заболеваемости и смертности, связанной с ИИСУ [<xref ref-type="bibr" rid="cit2">2</xref>]. Принятая Европейская система оценки сердечного риска шкала EuroSCORE II, позволяющая прогнозировать вероятность развития периоперационных осложнений, адаптирована и широко используется преимущественно для операций коронарного шунтирования и хирургического лечения пороков сердца [<xref ref-type="bibr" rid="cit6">6</xref>]. Доказательные подходы к снижению риска заражения при ИСУ включают предоперационное введение антибиотиков и тщательную антисептическую подготовку операционного поля и операционной бригады, задействованной непосредственно в операции [<xref ref-type="bibr" rid="cit2">2</xref>]. Было показано, что парентеральное введение антибиотиков перед процедурой имплантации значительно снижает частоту ИО [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit11">11</xref>]. В двойном слепом рандомизированном исследовании 649 пациентов были разделены на группы внутривенного введения цефазолина или плацебо, которые вводились непосредственно перед операцией [<xref ref-type="bibr" rid="cit11">11</xref>]. Данное исследование было прекращено досрочно из-за явных доказательств пользы; ИО развились у 0,63% в группе цефазолина и 3,28% в группе плацебо (относительный риск 0,19; p=0,016). На основании этого знакового исследования Американская кардиологическая ассоциация рекомендует назначать парентеральные антибиотики в течение часа до процедуры ИСУ [<xref ref-type="bibr" rid="cit2">2</xref>]. Антисептика кожи является еще одним стандартным предоперационным методом снижения риска ИИСУ [<xref ref-type="bibr" rid="cit2">2</xref>]. Некоторые данные свидетельствуют о преимуществах использования хлоргексидина-спирта по сравнению с повидон-йодом у пациентов, перенесших в подавляющем большинстве абдоминальные и торакальные "чистые" операции (не связанные с ИСУ) [<xref ref-type="bibr" rid="cit12">12</xref>]. В данной работе сообщалось о значительно более низком уровне инфицирования при использовании хлоргексидин-спирта по сравнению с повидон-йодом (9,5% против 16,1%; p=0,004). Другие распространенные профилактические методы включают стабилизацию клинического состояния пациента, использование скраба с хлоргексидином (Hibiclens) перед процедурой, уничтожение назальных бактерий, устойчивых к метициллину S. aureus (MRSA) и промывание ложа ИСУ антибиотиками. Однако стоит отметить, что данные методы не имеют абсолютную, доказанную эффективность для профилактики ИИСУ [<xref ref-type="bibr" rid="cit13">13</xref>]. Также в настоящее время отсутствуют убедительные данные в поддержку использования антибиотиков в послеоперационном периоде [<xref ref-type="bibr" rid="cit9">9</xref>]. Несмотря на минимальную доказательную базу, эти стратегии широко распространены в клинической практике. В большой когорте из 6983 больных, участвовавших в исследовании WRAP-IT, 74,5% пациентам промывали ложе ИСУ антимикробными растворами, а 29,6% больным назначались антибиотики после процедуры [<xref ref-type="bibr" rid="cit3">3</xref>].</p></sec><sec><title>Чехол имплантируемых сердечных устройств с покрытием антибиотиками</title><p>Чехлы ИСУ с антимикробной пропиткой были разработаны для надежного удержания устройств на месте, обеспечения стабильной среды и снижения риска ИО [<xref ref-type="bibr" rid="cit14">14</xref>]. Вместе с тем, они обеспечивают пролонгированную и высокую концентрацию антибиотиков, эффективных в отношении распространенных патогенов при ИСУ. Поскольку считается, что большинство ИИСУ происходит во время имплантации, эти чехлы стали эффективным методом профилактики. Данные других хирургических процедур подтверждают эффективность местного введения антибиотиков для предотвращения раневых инфекций [15-17]. Например, использование коллагеновых имплантатов, пропитанных гентамицином, значительно снижает риск стернальных раневых инфекций у пациентов из группы высокого риска, перенесших кардиохирургические операции [16-18]. Исследования этой технологии на основе коллагена выявили высокие концентрации гентамицина в месте хирургического вмешательства и очень низкие уровни препарата в сыворотке крови, что позволяет предположить, что антибиотик концентрируется в месте потенциальной инфекции, сводя при этом к минимуму системное воздействие и риск побочных эффектов или развития бактериальной резистентности [17-20].</p><p>Оба чехла ИСУ в настоящее время доступны для использования в США. Они являются резорбируемыми. Один из них изготовлен из рассасывающегося мультифиламентного блок-сополимера, состоящего из гликолида, капролактона и сетки триметиленкарбоната, который покрыт рассасывающимся полиакрилатным полимером, содержащим миноциклин и рифампицин. Второй чехол состоит из децеллюляризованного несшитого ВКМ, полученного из подслизистой оболочки кишечника свиньи. Перед имплантацией это устройство гидратируют в растворе гентамицина. Эти чехлы предназначены для стабилизации ИСУ внутри ложа, обеспечивают субстрат для врастания тканей и местную доставку антибиотиков, снижая риск ИО.</p></sec><sec><title>Чехол из синтетической сетки</title><p>Чехол ИСУ, изготовленный из синтетической сетки (TYRX™; Medtronic, Inc., Monmouth Junction, NJ), элюирует миноциклин и рифампицин в течение как минимум семи дней. Предыдущая версия чехла (AEGIS) состояла из нерассасывающегося полипропилена, что вызывало значительный фиброз карманов и, соответственно, в дальнейшем затрудняло удаление ИСУ и/или электродов. В связи с этим было разработано следующее поколение чехла, который полностью рассасывается примерно через девять недель после имплантации. Первоначальные ретроспективные исследования показали потенциальные его преимущества, а полученные результаты были дополнительно подтверждены проспективными исследованиями CITADEL и CENTURION [21-23]. Комбинированный анализ полученных данных выявил значительно более низкую частоту ИИСУ среди включенных пациентов, у которых использовался чехол, по сравнению с контрольной группой (0,4% против 2,2%; p=0,0023) [<xref ref-type="bibr" rid="cit23">23</xref>]. Недавний мета-анализ пяти когортных исследований показал относительный риск 0,31 [ 95% доверительный интервал (ДИ) 0,17-0,58; p=0,0002] для развития ИИСУ с использованием ЧВА и без него [<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>Важно отметить, что в проспективном многоцентровом рандомизированном клиническом исследовании (РКИ) WRAP-IT 2019 была продемонстрирована эффективность синтетической оболочки [<xref ref-type="bibr" rid="cit3">3</xref>]. В этом РКИ приняли участие 6983 пациента после первичной имплантации, замены или ревизии ИСУ, разделенных на группы с ЧВА и без него (контроль). По результатам РКИ в основной и контрольной группах 25 и 42 больных соответственно достигли первичной конечной точки (ИО, приведшее к деимплантации или ревизии системы устройство-электрод, длительной антибиотикотерапии с рецидивом инфекции или смерти) в течение 12 мес после процедуры. Расчетная частота событий составила 0,7% против 1,2% соответственно (p=0,04). Кроме того, долгосрочное наблюдение (36 мес) в WRAP-IT показало, что серьезные ИИСУ, произошли у 32 пациентов с ЧВА и у 51 больного из контрольной группы [оценка Каплана-Мейера 1,3% против 1,9%; отношение рисков (ОР) 0,64; 95% ДИ 0,41-0,99; p=0,046]. Любая другая инфекция, связанная с ИСУ, развилась у 57 пациентов с ЧВА и 84 больных из контрольной группы (оценка Каплана-Мейера 2,1% против 2,8%; ОР 0,69; 95% ДИ 0,49-0,97; p=0,030). Осложнения, связанные с системой или операцией, возникли у 235 пациентов с ЧВА и 252 больных из контрольной группы (оценка Каплана-Мейера 8,0% против 8,2%; ОР 0,95; 95% ДИ 0,79-1,13; p&lt;0,001 для не меньшей эффективности) [<xref ref-type="bibr" rid="cit25">25</xref>]. Хотя результаты этого хорошо спланированного РКИ (WRAP-IT) достигли статистической значимости (p&lt;0,05), они оказались не такими впечатляющими, как хотелось, из-за более низкого, чем ожидалось, общего уровня инфицирования. Более того, число пациентов, нуждающихся в лечении для предотвращения одной ИИСУ, составило 200 больных, что является высоким значением по сравнению с другими профилактическими методами. Таким образом, необходимы дальнейшие научные исследования и усовершенствование имеющихся технологий ЧВА для улучшения клинических преимуществ и снижения затрат.</p></sec><sec><title>Чехол внеклеточного матрикса</title><p>В настоящее время доступен еще один чехол для ИСУ, в котором используются другие материалы и антибиотики. Чехол CanGaroo-G™ (Aziyo Biologics, Silver Spring, MD) изготовлен из биологического материала, в котором используется ВКМ, а не синтетическая сетка, для создания биокаркаса для интеграции тканей и неоваскуляризации. Перед имплантацией оболочку ВКМ гидратируют в растворе гентамицина (20 мл раствора 40 мг/мл) в течение двух минут. Чехол ВКМ можно использовать для трансвенозных ИСУ, а также для подкожно имплантируемых кардиовертеров-дефибрилляторов. K. Xiang и соавт. представили первое опубликованное клиническое процедурное использование чехла для подкожно имплантируемых кардиовертеров-дефибрилляторов [<xref ref-type="bibr" rid="cit26">26</xref>]. Доклинические исследования показывают, что оболочка ВКМ доставляет высокие концентрации гентамицина в окружающие ткани с минимальным системным воздействием, обеспечивая превосходную эффективность против Staphylococcus spp. и других возбудителей ИИСУ [<xref ref-type="bibr" rid="cit27">27</xref>]. Важно отметить, что высвобождение гентамицина из оболочки ВКМ достигает пика довольно быстро (в течение одного часа), за которым следует постепенное, устойчивое высвобождение в течение одной недели. Этот фармакокинетический профиль соответствует предполагаемому механизму ИИСУ, который чаще всего включает загрязнение ложа кожной флорой во время имплантации. В недавно опубликованной модели доклинического заражения животных ни в одной культуре не было обнаружено бактерий после 12 ч воздействия оболочки ВКМ, содержащей гентамицин. Уровни гентамицина в сыворотке упали ниже предела количественного определения через 15 ч после имплантации [<xref ref-type="bibr" rid="cit28">28</xref>]. Концентрация гентамицина в оболочке внеклеточного матрикса оставалась относительно стабильной в течение семи дней [<xref ref-type="bibr" rid="cit28">28</xref>]. Эти результаты позволяют предположить, что оболочка ВКМ, содержащая гентамицин, эффективно снижает бактериальную нагрузку в ложе имплантата.</p></sec><sec><title>Синтетические и биологические материалы</title><p>Было показано, что по сравнению с искусственными материалами, такими как синтетическая хирургическая сетка, биологический ВКМ способствует большей интеграции тканей и врастанию сосудов, уменьшению воспалительной реакции и более быстрому выведению бактерий [29-34]. Многочисленные исследования продемонстрировали, что материалы на основе ВКМ, особенно нетканые, способствуют функциональному ремоделированию тканей в зависимости от места [<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit30">30</xref>]. Также было показано, что нетканые биологические материалы минимизируют реакцию инородного тела, воспаление и фиброз, а также способствуют ангиогенезу и дифференцировке клеток как часть процесса ремоделирования [<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. В результате уменьшения воспаления, усиления врастания тканей и ангиогенеза эти биологические материалы могут способствовать уничтожению бактерий и, тем самым, снижать риск заражения [<xref ref-type="bibr" rid="cit33">33</xref>]. Более того, некоторые исследования показывают, что биокаркасы ВКМ обладают собственной бактерицидной активностью [<xref ref-type="bibr" rid="cit34">34</xref>]. Некоторые антибактериальные факторы активны в интактном ВКМ, тогда как другие высвобождаются во время модификации матрикса после имплантации. Было высказано предположение, что компоненты интактного ВКМ способствуют трансформации фенотипа макрофагов от преимущественно провоспалительного (так называемого М1) к противовоспалительному (М2). Макрофаги типа М1 опосредуют повреждение тканей и способствуют воспалительной реакции [<xref ref-type="bibr" rid="cit35">35</xref>]. Было показано, что инфильтрация макрофагами типа М2 происходит на ранних стадиях восстановления тканей, а истощение макрофагов типа М2 ингибирует образование васкуляризированных грануляционных и рубцовых тканей [<xref ref-type="bibr" rid="cit36">36</xref>]. Материалы для восстановления с различными характеристиками стимулируют ряд реакций макрофагов. В исследовании была продемонстрирована корреляция между ранним ответом макрофагов и результатами ремоделирования тканей [<xref ref-type="bibr" rid="cit37">37</xref>]. Большая доля макрофагов М2 связана с положительными результатами ремоделирования, как это происходит при нетканом ВКМ. Напротив, синтетические и биологические материалы с высокой степенью поперечного плетения обычно вызывают более сильную реакцию инородного тела, включая хроническое воспаление и фиброзную инкапсуляцию материала, а не интеграцию в ткани хозяина [<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Воспалительная реакция на синтетические и тканые материалы характеризуется преобладанием макрофагов фенотипа М1, что способствует образованию рубцовой ткани и инкапсуляции материала [<xref ref-type="bibr" rid="cit36">36</xref>][<xref ref-type="bibr" rid="cit38">38</xref>]. Основываясь на этих данных, материалы для биологического восстановления обычно предпочтительнее синтетических в загрязненных хирургических полях [<xref ref-type="bibr" rid="cit39">39</xref>].</p></sec><sec><title>Применяемые антибиотики</title><p>Наблюдательные исследования ряда хирургических вмешательств показали снижение уровня инфицирования при местном применении антибиотиков. В современных ЧВА при ИСУ для профилактики ИО используются либо рифампицин/миноциклин, либо гентамицин.</p><p>Рифампицин и миноциклин</p><p>Рифампицин ингибирует бактериальный ДНК-зависимый синтез РНК, тогда как миноциклин ингибирует синтез бактериального белка. Оба препарата активны в отношении Staphylococcus spp. [<xref ref-type="bibr" rid="cit9">9</xref>]. Сообщаемая минимальная ингибирующая концентрация (МИК) рифампицина для S. aureus и коагулазонегативных стафилококков, выделенных при ИИСУ, составляет от ~0,5 до 2 мг/л [<xref ref-type="bibr" rid="cit40">40</xref>]. Исследования in vitro продемонстрировали, что рифампицин обладает активностью против биопленок S. epidermidis, особенно в сочетании с некоторыми другими агентами, тогда как многие другие протестированные антибиотики оказались неэффективными [<xref ref-type="bibr" rid="cit41">41</xref>]. Было показано, что добавление рифампицина к миноциклину усиливает его антибактериальный эффект in vitro, в том числе на резистентные штаммы, такие как MRSA [<xref ref-type="bibr" rid="cit42">42</xref>]. Комбинация обоих препаратов также снижает устойчивость к рифампицину [<xref ref-type="bibr" rid="cit43">43</xref>].</p><p>Комбинация рифампицина/миноциклина широко используется в катетерах, пропитанных антибиотиками. Результаты исследований свидетельствуют о значительном снижении риска инфицирования при использовании катетеров, пропитанных рифампицином/миноциклином, по сравнению со стандартными катетерами при нейрохирургических процедурах [<xref ref-type="bibr" rid="cit44">44</xref>], проведении химиотерапии [<xref ref-type="bibr" rid="cit45">45</xref>] и других применениях центральных венозных катетеров [<xref ref-type="bibr" rid="cit46">46</xref>]. Исследования ЧВА из синтетической сетки, элюирующей комбинацию рифампицина и миноциклина, продемонстрировали снижение бактериального загрязнения на моделях кроликов, включая устранение видов Staphylococcus, Escherichia coli и других видов бактерий [<xref ref-type="bibr" rid="cit47">47</xref>]. Вышеупомянутое исследование WRAP-IT продемонстрировало значительное (p&lt;0,05) снижение числа ИО, связанных с ИСУ, при использовании синтетической оболочки элюирующей рифампицин/миноциклин по сравнению с контрольной группой, где она не использовалась [<xref ref-type="bibr" rid="cit3">3</xref>].</p><p>Гентамицин</p><p>Гентамицин представляет собой аминогликозид, который ингибирует синтез бактериального белка и обладает бактерицидной активностью широкого спектра, включая Staphylococcus spp. и аэробные грамотрицательные микроорганизмы. Исследования чувствительности демонстрируют логарифмическое снижение бактериальной колонизации (т.е. снижение &gt;99,9%) как in vitro, так и in vivo, включая бактерии, которые чаще всего выделяются при ИО, связанных с ИСУ. Сообщается, что МИК гентамицина для распространенных возбудителей ИСУ (например, Staphylococcus spp.) составляет от ~0,5 до 4 мг/л [<xref ref-type="bibr" rid="cit40">40</xref>][<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>Гентамицин продемонстрировал эффективность in vitro против стафилококковых биопленок в одном исследовании, только даптомицин имел аналогичную эффективность среди монокомпонентных антибиотиков [<xref ref-type="bibr" rid="cit49">49</xref>]. Аналогично, фармакодинамическое исследование in vitro продемонстрировало логарифмическое снижение количества инокулятов S. aureus в шестикратном диапазоне концентраций гентамицина [<xref ref-type="bibr" rid="cit50">50</xref>].</p><p>Некоторые клинические данные и исследования на животных также свидетельствуют о противомикробной эффективности гентамицина для профилактики ИИСУ [<xref ref-type="bibr" rid="cit28">28</xref>]. Было показано, что местное использование гентамицина снижает уровень инфицирования у пациентов, перенесших имплантацию ИСУ [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit51">51</xref>].</p><p>Основным ограничением системного применения гентамицина является его связь с риском нефротоксичности и ототоксичности. Системное введение аминогликозидов использовалось нечасто в течение нескольких десятилетий, тем самым ограничивая развитие резистентности к этому классу антибиотиков. И, наоборот, как описано выше, местное введение гентамицина продемонстрировало эффективность при многочисленных хирургических процедурах без риска системного воздействия [16-18][<xref ref-type="bibr" rid="cit24">24</xref>][<xref ref-type="bibr" rid="cit51">51</xref>].</p><p>Эффекты гентамицина зависят от концентрации: для обеспечения высокой бактерицидной активности требуется высокая концентрация в области хирургического вмешательства, даже в отношении организмов с низкой чувствительностью или устойчивостью к этому агенту. При введении в область хирургического вмешательства концентрация гентамицина в местных тканях может превышать 300 мг/л, тогда как системное воздействие ограничивается ~1-2 мг/л [<xref ref-type="bibr" rid="cit19">19</xref>]. В одном исследовании раневой инфекции грудины лечение с использованием коллагенового имплантата, наполненного гентамицином, было эффективным даже при наличии устойчивого к антибиотикам S. epidermidis, предположительно из-за высоких местных концентраций гентамицина [<xref ref-type="bibr" rid="cit19">19</xref>]. Таким образом, местное введение гентамицина с использованием оболочки ВКМ обеспечивает высокие ранние концентрации антибиотиков в тканях при минимальном системном воздействии.</p><p>В настоящее время в доклинических моделях заражения животных, а также в отчетах о клинических случаях, описывается эффективность гентамицина в составе чехла ВКМ [<xref ref-type="bibr" rid="cit26">26</xref>][<xref ref-type="bibr" rid="cit28">28</xref>]. Тем не менее, необходимо проспективное РКИ, чтобы доказать безопасность и эффективность снижения ИИСУ. Стоит отметить, что аналогичная доклиническая модель на животных использовалась ранее для исследования безопасности и эффективности синтетической оболочки миноциклин/рифампицин [<xref ref-type="bibr" rid="cit47">47</xref>]. Результаты таких экспериментов на животных позже были подтверждены в проспективном РКИ [<xref ref-type="bibr" rid="cit3">3</xref>].</p></sec><sec><title>Заключение</title><p>Минимизация рисков неблагоприятных исходов, связанных с ИСУ, особенно ИО, имеет первостепенное значение для предотвращения серьезных осложнений, которые требуют удаления ИСУ. Новые технологии чехлов обещают снизить вероятность заражения ложа ИСУ. Доклинические и клинические данные подтверждают эффективность местной доставки антибиотиков через синтетические оболочки или оболочки ИСУ на основе ВКМ, элюирующие рифампицин/миноциклин и гентамицин соответственно. Более того, оболочка ВКМ обычно стимулирует ремоделирование тканей и ангиогенез, тем самым сводя к минимуму воспаление и способствуя клиренсу бактерий.
