Однако у подавляющего большинства пациентов с длительной ГУ подагра никогда не развивается. Точно также, гиперхолестеринемия, доказанный фактор атерогенеза, далеко не у всех пациентов приводит к развитию соответствующих клинических событий. Это общность может быть объяснена вовлечением универсального механизма воспаления. Ключевым медиатором острых приступов подагры признан интерлейкин (IL)-1β, продукт активации инфламмасомы NLRP3 (NLR family pyrin domain containing 3), сложного мультипротеинового комплекса, ответственный за местную воспалительную реакцию в синовиальной оболочке и периартикулярных тканях с участием макрофагов и нейтрофилов. Активация инфламмасомы NLRP3 осуществляется кристаллами мочевой кислоты (МК), холестерина исключительно после прайминга липополисахаридами, продуктами перекисного окисления и другими факторами повреждения, ассоциированными со старением и коморбидными состояниями, типичными для подагры и сердечно-сосудистых заболеваний. К тому же активность инфламмасомы NLRP3 генетически детерминирована и определяет частоту этих состояний. Обсуждаемый механизм объясняет, почему воздействие на факторы, связанные с коморбидностью, способно снизить частоту приступов подагры наряду с сердечно-сосудистыми исходами. В статье показаны новые клинически значимые плейотропные эффекты статинов и ингибиторов натрий-глюкозного транспортера 2 типа, которые имеют преимущества перед уратснижающей терапией у пациентов с бессимптомной ГУ и способны модифицировать течение подагры. Подчеркиваются их противовоспалительные свой­ства, кардио- и ренопротективные эффекты, пре­имущества в переносимости. Блокирование активности инфламмасомы рассматривается как новая универсальная для ревматологии и кардиологии терапевтическая мишень, особенно при состояниях повышенного сердечно-сосудистого риска, ассоциированного с ГУ.</p></abstract><trans-abstract xml:lang="en"><p>From modern perspective, hyperuricemia should be considered as a trigger of inflammatory activity in tissues and organs, leading to the formation of tophi, arthropathy, kidneys and cardiovascular system damage. Similarly, hypercholesterolemia, a proven factor in atherogenesis-far from all patients leads to the development of relevant clinical events. This commonality may be explained by the involvement of universal inflammatory mechanism. The key mediator of gout attacks is recognized as IL-1β, a product of NLRP3 (NLR family pyrin domain containing 3) inflammasome activation (complex multiprotein), responsible for local inflammatory response in synovial membrane and periarticular tissues with participation of macrophages and neutrophils. NLRP3 inflammasome activation is carried out by uric acid crystals, cholesterol exclusively after priming by lipopolysaccharides, peroxidation products and other damage factors associated with aging and comorbid conditions typical for gout and cardiovascular diseases. In addition, NLRP3 inflammasome activity is genetically determined and determines the frequency of these conditions. The discussed mechanism explains why the impact on factors associated with comorbidity is able to reduce the frequency of gout attacks along with cardiovascular outcomes. New clinically relevant pleotorpic effects of statins, sodium-glucose cotransporter-2 inhibitors, which have advantages over urates-lowering therapy in patients with asymptomatic hyperuricemia and can modify the course of gout, are demonstrated. Their anti-inflammatory properties, cardio and renoprotective effects, and tolerability advantages are emphasized. Blocking the activity of inflammasome is considered as a new universal therapeutic target for rheumatology and cardiology.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>инфламмасома NLRP3</kwd><kwd>гиперурикемия</kwd><kwd>подагра</kwd><kwd>сердечно-сосудистые заболевания</kwd><kwd>ингибиторы натрий глюкозного транспортера 2 типа</kwd><kwd>статины</kwd></kwd-group><kwd-group xml:lang="en"><kwd>NLRP3 inflammasome</kwd><kwd>hyperuricemia</kwd><kwd>gout</kwd><kwd>cardiovascular diseases</kwd><kwd>sodium-glucose cotransporter-2 inhibitors</kwd><kwd>statins</kwd></kwd-group></article-meta></front><body><sec><title>Введение</title><p>Мочевая кислота (МК) — конечный продукт пуринового метаболизма независимо от алиментарного или эндогенного источника пуринов. У млекопитающих МК расщепляется уриказой, которая не экспрессируется у человека. Поэтому концентрация МК в крови у людей увеличена более чем в 50 раз в сравнении с животными [<xref ref-type="bibr" rid="cit1">1</xref>], что и создает риск ассоциированных с гиперурикемией (ГУ) заболеваний и состояний — подагры, уратной нефропатии, обменных нарушений, изменения костного метаболизма, увеличения сердечно-сосудистого (СС) риска (ССР). ГУ отражает содержание МК и уратов в сыворотке крови выше 6 мг/дл (~360 мкмоль/л) для женщин и 7 мг/дл (~420 мкмоль/л) для мужчин, при котором в физиологических условиях происходит ее кристаллизация [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Хотя ГУ повышает риск подагры пропорционально значениям концентрации МК крови, тем не менее, ее высокие концентрации у подавляющего большинства пациентов бессимптомны, в том числе и в ситуации, при которой доказано отложение кристаллов моноурата натрия в суставном хряще и тканях сухожилий. Для этих случаев используется термин бессимптомная ГУ (БГУ). Хотя клинические проявления подагры могут развиться у человека с ГУ в любой момент, примерно две трети или более таких людей остаются бессимптомными, у них никогда не развиваются острые приступы подагрического артрита, тофусная подагра, острая или хроническая уратная нефропатия или нефролитиаз.</p><p>В последние десятилетия активно обсуждается связь БГУ с другими состояниями, патогенез которых не связан с образованием кристаллов уратов, например, артериальной гипертензией, ишемической болезнью сердца (ИБС), хронической болезнью почек (ХБП) и синдромом резистентности к инсулину [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Причем, выявленная связь между МК крови и СС событиями определяется даже при уровнях МК ниже предела растворимости уратов [<xref ref-type="bibr" rid="cit5">5</xref>]. Актуальность проблематики ГУ для кардиологии усиливается ее значительной распространенностью, пропорциональной возрасту. В общей популяции РФ в диапазоне 25-64 года ГУ выявлена у 16,8% [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>В основе современного понимания сердечно-сосудистых заболеваний (ССЗ) лежат по крайней мере три взаимодополняющие концепции, основанные на роли холестерина, дисфункции сосудистого эндотелия и низкоинтенсивной воспалительной реакции в сосудистой стенке [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>Наряду с атеросклерозом, наиболее распространенными ассоциированными с возрастом заболеваниями считаются артериальная гипертензия, сахарный диабет 2 типа (СД 2) и подагра. Сочетание этих болезней у одного пациента настолько типично, что рассматривается как закономерность [<xref ref-type="bibr" rid="cit9">9</xref>]. Парадигма лечения таких пациентов может быть определена как коррекция каждого биомаркера до целевого уровня: холестерина, артериального давления, гликированного гемоглобина, МК крови. Причем цели должны достигаться специфичными для каждого параметра препаратами, как это описано в соответствующих рекомендациях.</p><p>Впервые связь МК крови с ССР установлена в популяции Фремингемского исследования в 1967 г. Метаанализ проспективных когортных исследований показал, что увеличение МК на каждые 1 мг/дл (60 мкмоль/л) увеличивает риск ИБС на 20% и риск общей смертности на 9% [<xref ref-type="bibr" rid="cit10">10</xref>]. Эти и последующие исследования предоставили данные о том, что БГУ может быть независимым фактором риска ССЗ, и это положение закреплено в современных рекомендациях, например, у пациентов с АГ [<xref ref-type="bibr" rid="cit11">11</xref>]. Так же, как БГУ, подагра по данным крупных эпидемиологических исследований ассоциирована с увеличением частоты СС и почечных исходов [12-14]. Среди многочисленных механизмов, которые могут лежать в основе этой взаимосвязи, системная воспалительная реакция представляется наиболее важной, поскольку именно она сопряжена с коморбидностью [<xref ref-type="bibr" rid="cit15">15</xref>] и сопровождает другие артриты и спондилоартропатии, характеризующиеся повышенным риском ССЗ [<xref ref-type="bibr" rid="cit16">16</xref>].</p></sec><sec><title>Атеросклероз — подагра коронарных артерий?</title><p>Влияние МК на ССЗ вполне может объясняться установленной связью с атерогенными липидными параметрами [<xref ref-type="bibr" rid="cit16">16</xref>]. Причины таких взаимоотношений многообразны, среди них генетические факторы, а также инсулинорезистентность, вызывающая снижение расчетной скорости клубочковой фильтрации (рСКФ) и элиминации МК. Возможен и обратный эффект, учитывая то, что уратснижающая терапия (УСТ) способна положительно влиять на инсулинорезистентность при метаболическом синдроме [<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit18">18</xref>]. Все это указывает на переплетение факторов риска подагры и атеросклероза. Вероятно, что эти связи двунаправлены, поэтому УСТ может быть фактором снижения атерогенности, а снижение холестерина может способствовать профилактике подагры.</p><p>Примерно у 15% пациентов с БГУ и у 79-86% пациентов с подагрой методом двухэнергетической компьютерной томографии выявлены депозиты МК в кровеносных сосудах [<xref ref-type="bibr" rid="cit19">19</xref>]. У 30% пациентов с подагрой кристаллы уратов обнаруживаются в коронарных артериях этим методом. Их локализация часто соответствуют атеросклеротическим бляшкам и сопровождается кальцинацией [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Кристаллы холестерина, откладываясь в тканях сосудов из липопротеинов низкой плотности, вызывают местную воспалительную реакцию, свойственную атеросклерозу. Также выпадение кристаллов МК в суставной жидкости, хряще приводит не только к местной, но и системной воспалительной реакции, что наряду с накоплением кристаллов уратов в сосудистой стенке может способствовать образованию атеросклеротических бляшек. Это позволяет по-новому взглянуть на ИБС, представив ее как "подагру" коронарных артерий. С другой стороны, как далеко не у всякого пациента с гиперлипидемией разовьется какая-либо форма ИБС, так далеко не каждый пациент с ГУ испытает приступ подагры. Эта общая особенность обсуждаемых состояний показывает, что для развития болезней должен сложиться комплекс условий, который мы должны осмыслить. Действительно, общей для подагры и ИБС является системная воспалительная реакция, которая связана с активацией инфламмасомы и характеризуется повышением активности в крови IL-1β, IL-6, IL-18, способствуя образованию новых атеросклеротических бляшек и их дестабилизации [<xref ref-type="bibr" rid="cit22">22</xref>].</p></sec><sec><title>Активация инфламмасомы NLRP3 в каскаде системной воспалительной реакции</title><p>Интерлейкин (IL)-1β признан ключевым патогенетическим фактором острого приступа подагры. IL-1β действует на большинство типов клеток и обладает мощным воспалительным потенциалом, запуская несколько последующих путей воспаления через индукцию других цитокинов в клетках-мишенях, включая фактор некроза опухоли альфа, IL-6 и IL-8. Его образование происходит под действием каспазы-1, расщепляющей предшественник IL — 1β [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>В настоящее время подагра рассматривается как системное воспалительное заболевание, вызванное активацией инфламмасомы NLRP3 (NOD-подобного рецепторного белка 3) [<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>Инфламмасомы — это мультимерные белковые внутриклеточные платформы, которые активируются в два этапа в ответ на инфекцию или повреждение ткани. Наиболее изученной является инфламмасома NLRP3, сборка которой необходима для активации каспазы-1 [<xref ref-type="bibr" rid="cit25">25</xref>]. Воздействие на Toll-подобные рецепторы кристаллами МК (второй этап) приводит к активации инфламмасомы, но исключительно после ее сенсибилизации при воздействии так называемыми праймерами — липосахаридами или свободными жирными кислотами [<xref ref-type="bibr" rid="cit22">22</xref>]. Эти факторы известны как классические триггеры острого приступа подагры [<xref ref-type="bibr" rid="cit26">26</xref>][<xref ref-type="bibr" rid="cit27">27</xref>]. Только комбинация двух сигналов опосредует и индуцирует серию каскадных реакций подагрического воспаления при участии клеток крови — макрофагов, моноцитов и нейтрофилов [<xref ref-type="bibr" rid="cit27">27</xref>]. Изначально считалось, что МК вызывает воспаление через активацию инфламмасом NLPR3 только вследствие формирования кристаллов. Позднее было продемонстрировано, что растворимая МК, проникая в клетки через специальные транспортеры, может также активировать инфламмасомы NLPR3, вызывая высвобождение медиаторов воспаления [<xref ref-type="bibr" rid="cit27">27</xref>]. Инфламмасома NLRP3 экспрессируется большим набором клеток, включая моноциты, нейтрофилы, гладкомышечные клетки, эндотелиоциты, что косвенно свидетельствует о потенциальной вовлеченности различных тканей в воспалительную активность.</p><p>В генетическом анализе 8 однонуклеотидных последовательностей 7 разных генов NLRP3 в мексиканской популяции было выявлено влияние rs45520937 PPARGC1b на частоту подагры: отношение шансов (ОШ) =2,3; р=0,03 [<xref ref-type="bibr" rid="cit28">28</xref>]. Количество однонуклеотидных последовательностей генов NLRP3, ассоциированных с риском подагры, существенно пополнилось в последнее время [<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>Клиническая значимость блокады IL-1β как продукта активации инфламмасомы NLRP3 для купирования атак подагры имеет бесспорное подтверждение. Все три препарата, блокирующие биологическую активность IL-1β (канакинумаб, анакинра и рилонацеп 1) в метаанализе, включающем 4367 пациентов, показали высокую активность в купировании острого подагрического артрита в сравнении со стандартными противовоспалительными средствами, что позволяет рассматривать их в качестве замены при противопоказаниях к последним [<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>Активация инфламмасомы происходит в два этапа. Первичный сигнал действует через паттерн-узнающие рецепторы (Тоll-подобные) и активирует транскрипцию генов NLRP3 через ядерный фактор NF-kB, а также генов предшественников proIL-1β, proIL-18 и гасдермина D. Вторичный сигнал осуществляется продуктами гибели или стресса клеток DAMPs (danger-associated molecular patterns), которые в норме как внутриклеточные молекулы выполняют важные физиологические функции, но как внеклеточные молекулы выполняют роль активаторов инфламмасом (рис.). DAMPs активируют NLRP3-инфламмасому путем олигомеризации неактивной NLRP3, белка, ассоциированного с апоптозом (ASC) и прокаспазы-1. Этот комплекс в свою очередь катализирует конверcию прокаспазы-1 в каспазу-1, которая участвует в образовании и секреции ИЛ1β и ИЛ18 из соответствующих предшественников [<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit33">33</xref>]. DAMPs могут служить внеклеточные молекулы АТФ, МК, кристаллы различных веществ, включая холестерин и МК и т.д. Eсли сигнал 2 не подается, мРНК предшественников proIL-1β, proIL-18 и гасдермина D деградирует и активации инфламмасомы не происходит.</p><fig id="fig-1"><caption><p>Рисунок. Двухсигнальная активация инфламмасомы NLRP3 (рисунок создан авторами статьи с использованием источников [79][80])</p></caption><graphic xlink:href="rpcardio-20-3-g001.png"><uri content-type="original_file">https://cdn.elpub.ru/assets/journals/rpcardio/2024/3/25RMCT5fzNKo0kxKNdaVjG0EdHTUSiPXQsZJu4Rj.png</uri></graphic></fig></sec><sec><title>Антиподагрические эффекты новых ингибиторов инфламмасомы NLRP3</title><p>Хотя основным представителям УСТ аллопуринолу и фебуксостату приписываются многочисленные противовоспалительные свойства [<xref ref-type="bibr" rid="cit34">34</xref>], тем не менее, хорошо известно о фактах обострения подагры на фоне применения этих препаратов при некорректном назначении, например, сразу после приступа острого подагрического артрита. Именно поэтому большая востребованность существует в отношении не только способности препарата реализовать уратснижающий эффект, но и в его сочетании с реальной противовоспалительной активностью [<xref ref-type="bibr" rid="cit35">35</xref>]. К таким перспективным препаратам относится архалофенат, первоначально разрабатываемый как сенситизатор к инсулину благодаря его способности активировать рецепторы пролифератора пероксисомы (PPARγ). В последующем у него были выявлены урикозурические свойства за счет ингибирования уратных транспортеров URAT1, OAT4 и OAT10. Противовоспалительная активность архалофената реализуется несколькими путями, один из которых опосредован аденозин-монофосфат активирующей фосфокиназой через ингибицию ядерного фактора NF-kb и инфламмасому NLRP3 [<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Новый специфичный ингибитор инфламмасомы NLRP3 дапансутрил продемонстрировал высокую эффективность и приемлемый уровень безопасности в лечении острых приступов подагры в клиническом исследовании фазы IIА [<xref ref-type="bibr" rid="cit37">37</xref>].</p></sec><sec><title>Инфламмасома NLRP3 как перспективная терапевтическая мишень в кардиологии</title><p>Сосудистое воспаление играет важную роль в возникновении и прогрессировании атеросклеротических заболеваний. Оно характеризуется активацией моноцитов, которые, контактируя с эндотелием, проникают в субэндотелиальный слой, где взаимодействуют с эндогенными медиаторами, такими как модифицированные липопротеины, активирующие врожденный иммунный ответ, частью которого является инфламмасома NLRP3. Ее участие доказано в развитии атеросклероза, в том числе на животных моделях [<xref ref-type="bibr" rid="cit27">27</xref>]. Как показано выше, активация инфламмасомы осуществляется широким набором стимулов (в том числе и кристаллами холестерина). Очевидно, факторы сенсибилизации имеют связь с коморбидностью и процессом старения. Этот аспект позволяет объяснить, почему ГУ далеко не всегда вызывает подагру и почему снижение МК не имеет четкой связи с СС исходами. Следовательно, воздействие на внешние средовые факторы, и внутренние, ассоциированные с коморбидностью (перекиси, дислипидемия, жирные кислоты, DAMPs др.) само по себе способно оказывать антиподагрический и кардио-, ренопротективные эффекты [<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>Другим фактором, модулирующим воспалительную реакцию сосудистой стенки на ГУ, может быть различная, генетически опосредованная индивидуальная восприимчивость инфламмасомы к сенсибилизации. В крупном исследовании изучалась ассоциация генетических вариантов NLRP3 с частотой ССЗ и смертностью у 538 167 человек с помощью генно-ориентированного подхода без проведения множественного тестирования [<xref ref-type="bibr" rid="cit38">38</xref>]. Функциональная значимость однонуклеотидных полиморфизмов, влияющих на активацию инфламмасомы NLRP3, была оценена в моноцитах периферической крови. Генетический анализ выявил высоко распространенный (частота минорных аллелей 39,9%) интронный вариант rs10754555, влияющий на экспрессию гена NLRP3. У его носителей были значительно повышены уровни С-реактивного белка и амилоида А в сыворотке крови. У носителей аллеля G наблюдалась более высокая активация инфламмасомы NLRP3 в изолированных моноцитах человека. У носителей варианта rs10754555 распространенность ИБС была значительно выше, значимо выше был риск СС смерти в период наблюдения. Индукторы инфламмасомы (например, ураты, триглицериды, аполипопротеин С3) модулировали ассоциацию между rs10754555 и смертностью. Таким образом, получено свидетельство существенной роли генетически обусловленного системного воспаления в развитии ССЗ на уровне инфламмасомы NLRP3, что позволяет рассматривать ее как терапевтическую мишень [<xref ref-type="bibr" rid="cit38">38</xref>]. В недавних работах выявлен целый ряд аллелей, определяющих фенотип инфламмасомы NLRP3 как протективный в плане ССЗ, или сопряженный с негативным прогнозом [<xref ref-type="bibr" rid="cit39">39</xref>].</p><p>Также есть свидетельства участия NLRP3 инфламмасомы в патогенезе миокардиального повреждения. Например, показано роль растворимой МК сыворотки крови в концентрации, недостаточной для образования кристаллов, как фактора повреждения через активацию NLRP3 инфламмасомы в кардиомиоцитах [<xref ref-type="bibr" rid="cit40">40</xref>].</p></sec><sec><title>Кардиопротективные свойства препаратов, ингибирующих активность инфламмасомы</title><p>Колхицин, использующийся для лечения подагрического артрита — алкалоид, экстрагируемый из безвременника осеннего. Кроме антиподагрических свойств он обладает способностью уменьшать ССР у пациентов с подагрой [<xref ref-type="bibr" rid="cit41">41</xref>]. Колхицин — мощный ингибитор полимеризации клеточных канальцев. Микротрубочки отвечают за субклеточный транспорт белка, ассоциированного с апоптозом (ASC) и NLRP3 в макрофагах, и необходимы для правильной цитозольной активации компонентов инфламмасомы. Таким образом, молекулы, влияющие на функцию микротрубочек, могут потенциально изменять сборку и функционирование NLRP3 инфламмасомы.</p><p>В кардиологии препарат известен как эффективное средство при остром перикардите [<xref ref-type="bibr" rid="cit42">42</xref>]. Колхицин был первым противовоспалительным препаратом, доказавшим способность уменьшать СС осложнения у пациентов с ИБС [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>]. Исследование COLCOT показало, что у пациентов с острым инфарктом миокарда, которые были рандомизированы в группу колхицина в дополнение к стандартной терапии в течение первых 30 дней, уменьшилась частота неблагоприятных коронарных и церебральных атеротромботических событий по сравнению с плацебо при наблюдении в течение 1,9 года [<xref ref-type="bibr" rid="cit45">45</xref>].</p><p>В недавнем исследовании COCS короткий период назначения колхицина в низкой дозе уменьшал частоту постоперационной фибрилляции предсердий у пациентов, перенесших открытую операцию на сердце (ОШ = 0,51, р&lt;0,029) [<xref ref-type="bibr" rid="cit46">46</xref>].</p><p>Компоненты инфламмасомного пути NLRP3, такие как IL-1β, вполне могут быть использованы в качестве терапевтических мишеней. В исследовании CANTOS канакинумаб (моноклональное антитело к IL-1β) снизил остаточный риск ССЗ у пациентов, перенесших острый инфаркт миокарда и находившихся на должной терапии липидснижающими препаратами, одновременно уменьшив частоту новых случаев подагры [<xref ref-type="bibr" rid="cit46">46</xref>]. Кардиопротективные свойства препаратов, известных как антиподагрические с противовоспалительными свойствами, объясняющимися ингибированием инфламмасомы или блокированием биологического эффекта ее конечных продуктов — интерлейкинов, открывают новое направление в кардиологии.</p></sec><sec><title>Кардиопротективные и антиподагрические эффекты статинов</title><p>Статины — базовые липидснижающие препараты, используемыми для профилактики ССЗ, однако плейотропные эффекты статинов могут быть полезны и при других хронических заболеваниях. Так, в крупный метаанализ было включено 54 исследования, посвященных влиянию статинов на смертность: 21 — с исходными ССЗ, 6 — с ХБП, 6 — с хроническими воспалительными заболеваниями (аутоиммунные заболевания, подагра), 3 — с онкологическими и 18 — с другими заболеваниями. Риск смертности от всех причин значительно снижался у тех, кто принимал статины (ОШ = 0,72, 95% доверительный интервал (ДИ); 0,66-0,76). Снижение риска смертности было одинаковым в исследованиях, посвященных ССЗ (ОШ = 0,73, 95% ДИ; 0,66-0,76) и прочим (ОШ = 0,70, 95% ДИ; 0,67-0,79). Существенных различий в снижении риска между когортами с разными заболеваниями не было (p=0,179) [<xref ref-type="bibr" rid="cit48">48</xref>]. Следует напомнить, что терапия статинами должна быть назначена пациентам с высоким/очень высоким СС риском, определяемым по шкале SCORE2 без подагры. Таблица SCORE2, принятая Европейским обществом кардиологов в 2021 г, увеличивает в 13,5 раз количество пациентов с высоким и очень высоким рисками, в сравнении с ранее использовавшейся версией SCORE [<xref ref-type="bibr" rid="cit49">49</xref>].</p><p>Подагра, по современным представлениям, увеличивает расчетное значение ССР в 1.5 раза [<xref ref-type="bibr" rid="cit50">50</xref>]. Поэтому, на основе этого подхода любой пациент с подагрой старше 40 лет с большой вероятностью подлежит лечению статинами. Это положение следует приветствовать, тем более что отмечено увеличение продолжительности жизни пациентов с подагрой под влиянием терапии статинами.</p><p>В крупном многолетнем регистровом исследовании оценивался относительный риск смерти в зависимости от факта лечения статинами. Среди 17 018 пациентов с подагрой, начавших прием статинов, в течение 5,0 лет риск кумулятивной смертности был на 16% ниже (ОШ = 0,84, 95% ДИ: 0,79-0,89). Эта протективная связь была сильнее у лиц без предшествующих ССЗ (ОШ = 0,65 против 0,85; для взаимодействия p=0,02) [<xref ref-type="bibr" rid="cit51">51</xref>].</p><p>Также позитивный выраженный эффект статинов на кумулятивную летальность подтвержден у пациентов с ХБП от 3 стадии вплоть до терминальной [<xref ref-type="bibr" rid="cit52">52</xref>]. Это важно, поскольку ХБП является и причиной, и следствием подагрического интерстициального нефрита. В отличие от данных процитированного исследования [<xref ref-type="bibr" rid="cit52">52</xref>], конкретных сведений в отношении способности УСТ влиять на прогрессирование почечной дисфункции или смертности у пациентов с ГУ и ХБП, в настоящее время нет [<xref ref-type="bibr" rid="cit53">53</xref>]. Вопрос о том, могут ли статины уменьшать вероятность развития подагры, также претендует на новизну. Связь между применением статинов и риском развития подагры у пациентов с гиперлипидемией изучена в крупном популяционном исследовании [<xref ref-type="bibr" rid="cit54">54</xref>]. Сравнивались пациенты, регулярно принимающие статины, и две активные группы сравнения (пациенты с нерегулярным приемом статинов и использованием других липидснижающих препаратов). Регулярный прием статинов незначительно снижал риск развития подагры по сравнению с нерегулярным их приемом (ОШ = 0,95; 95% ДИ, 0,90-1,01) и приемом других липидснижающих средств (ОШ = 0,94; 95% ДИ, 0,84-1,04).</p><p>Длительность приема статинов более &gt;3 лет соответствовала снижению частоты острого подагрического артрита (ОШ = 0,76; 95% ДИ, 0,64-0,90) по сравнению с нерегулярным применением статинов и по сравнению с использованием других липидснижающих препаратов) (ОШ = 0,50; 95% ДИ, 0,37-0,68).</p><p>Поэтому защитное преимущество статинов в отношении подагрических приступов не реализуется при кратковременном их приеме, а наблюдается у тех, кто получает более высокие кумулятивные дозы или имеет большую продолжительность терапии [<xref ref-type="bibr" rid="cit54">54</xref>]. Механизм подобной взаимосвязи изучен мало, и представляется логичным связать его с хорошо доказанной противовоспалительной активностью этой группы препаратов, в частности, подавлением активности инфламмасомы NLRP3, что сопровождается уменьшением IL1β и IL18 [<xref ref-type="bibr" rid="cit55">55</xref>]. Возможна также связь со способностью статинов — аторвастатина и симвастатина — уменьшать ГУ, что было показано в двух работах [<xref ref-type="bibr" rid="cit56">56</xref>][<xref ref-type="bibr" rid="cit57">57</xref>]. Необходимость терапии статинами у пациентов с подагрой и с БГУ подтверждается высокой распространенностью атерогенной дислипидемии у них [<xref ref-type="bibr" rid="cit58">58</xref>]. Надо сказать, что другие липидснижающие препараты, например, бемпедоевая кислота, существенно проигрывают статинам у пациентов с высоким ССР. В недавнем исследовании пациентов с высоким ССР и непереносимостью статинов включение препарата бемпедоевой кислоты в 1,94 раза увеличивало частоту ГУ, в 1,5 раз увеличивало риск случаев подагры и в 1,34 раза риск ухудшения почечной функции [<xref ref-type="bibr" rid="cit59">59</xref>].</p></sec><sec><title>Гипоурикемические и антиподагрические эффекты ингибиторов натрий-глюкозного транспортера 2 типа (иНГЛТ2)</title><p>иНГЛТ2 в клинической практике дебютировали в качестве сахароснижающих препаратов, поскольку обладают возможностью снижать концентрацию глюкозы крови в степени, сопоставимой с другими гипогликемическими препаратами. Однако выдающиеся кардио- и ренопротективные свойства этих, по сути, метаболических препаратов, выходят за рамки СД 2. В настоящее время возможность снижать МК крови иНГЛТ2 хорошо изучена и оценивается как один из плейотропных эффектов, присущих этой группе [<xref ref-type="bibr" rid="cit60">60</xref>]. У пациентов с СД 2 показано значимое снижение концентрации МК на рекордные 27% под влиянием дапаглифлозина 10 мг/сут. в течение недели, что коррелировало с увеличением фракции ее экскреции с мочой, дополнительно увеличивалась чувствительность к инсулину и функция β-клеток поджелудочной железы [<xref ref-type="bibr" rid="cit61">61</xref>]. Однако есть данные о том, что способность снижать концентрацию МК крови у пациентов с СД 2 под действием иНГЛТ2 меньше, чем у пациентов без диабета: на 31,48 мкмоль/л и 91,38 мкмоль/л, соответственно [<xref ref-type="bibr" rid="cit62">62</xref>].</p><p>Назначение иНГЛТ2 пациентам с СД 2 и ССЗ приводило к значительному уменьшению активности инфламмасомы и снижению IL1β. Эти препараты, вызывая глюкозурию, способствуют уменьшению доступности глюкозы как источника энергии, липолизу в адипоцитах и продукции кетонов печенью, что сопровождается образованием гидроксибутировой кислоты, обладающей ингибирующим действием на инфламмасому [<xref ref-type="bibr" rid="cit63">63</xref>]. Установлены и другие механизмы ингибирования инфламмасомы NLRP3 этими препаратами, в частности, блокада пути активные формы кислорода — NLRP3-каспаза-1 в макрофагах и снижение выработки IL-1β и IL-18 [<xref ref-type="bibr" rid="cit64">64</xref>]. Конечно, подагра не является показанием к назначению дапаглифлозина. Однако показанием к его назначению у пациента с подагрой или БГУ без СД 2, без хронической сердечной недостаточности (ХСН), может быть фактор сниженной рСКФ как признак ХБП. Это важно, поскольку подагра сопутствует ХБП в 25% случаев, а ГУ выявляется у 60% таких пациентов [<xref ref-type="bibr" rid="cit65">65</xref>]. На основе крупного рандомизированного клинического исследования (РКИ), доказавшего ренопротективные эффекты дапаглифлозина DAPA-CKD [<xref ref-type="bibr" rid="cit66">66</xref>], в 2021 г. официально утверждено показание к его применению: "ХБП у взрослых пациентов с риском ее прогрессирования для уменьшения риска устойчивого снижения рСКФ, наступления терминальной стадии хронической почечной недостаточности, смерти от ССЗ и госпитализации по поводу сердечной недостаточности" 2 [<xref ref-type="bibr" rid="cit67">67</xref>].</p></sec><sec><title>От ингибиторов ксантиноксидазы до иНГЛТ2 в лечении пациентов с ХСН и гиперурикемией</title><p>Другим частым коморбидным с ГУ и подагрой синдромом признана ХСН [<xref ref-type="bibr" rid="cit67">67</xref>]. Более того, ГУ может считаться патогенетическим фактором ХСН [<xref ref-type="bibr" rid="cit68">68</xref>]. Доказано, что ГУ коррелирует со значительным возрастанием риска развития ХСН, и высокий уровень МК в сыворотке является независимым предиктором смерти у пациентов с ХСН [<xref ref-type="bibr" rid="cit69">69</xref>].</p><p>Эти наблюдения стали предпосылками для изучения роли УСТ при ХСН. Однако, в двух наиболее известных из них РКИ: OPT-HF и EXACT-HF не выявлено преимуществ этих препаратов ни в уменьшении смертности, ни в улучшении качества жизни [<xref ref-type="bibr" rid="cit70">70</xref>][<xref ref-type="bibr" rid="cit71">71</xref>]. Как указано выше, новой ступенью в лечении ХСН, в том числе у пациентов с ГУ, оказался новый класс препаратов — иНГЛТ2. В большом метаанализе, включавшем 11005 пациентов с ХСН независимо от ФВЛЖ и коморбидности по СД 2 исследован эффект дапаглифлозина (РКИ APA-HF) [<xref ref-type="bibr" rid="cit72">72</xref>]. Распространенность подагры составила 10,1%. У пациентов с подагрой первичная конечная точка, которая включала СС смерть и госпитализации по поводу ХСН, наблюдалась чаще (ОШ = 1,15; 95% ДИ, 1,01-1,31).</p><p>Добавление дапаглифлозина в дозе 10 мг в сутки статистически значимо уменьшило риск первичной конечной точки в одинаковой степени у пациентов с подагрой (ОШ = 0,84; 95% ДИ, 0,66-1,06) и без нее (ОШ = 0,79; 95% ДИ, 0,71-1,06). Важно отметить, что прием дапаглифлозина снижал потребность в терапии гипоурикемическими препаратами (ОШ = 0,43; 95% ДИ, 0,34-0,53) и колхицином (ОШ = 0,54; 95% ДИ, 0,37-0,80) [<xref ref-type="bibr" rid="cit73">73</xref>].</p><p>Очевидно, что новая группа препаратов демонстрирует другой уровень возможностей в сравнении с классическим ингибиторами ксантиноксидазы именно за счет множественных плейотропных свойств. Возможность уменьшать главные СС исходы у пациентов с СД 2, уникальный ренопротективный эффект, независимый от гликемического статуса, способность профилактировать осложнения ХСН при любом ее фенотипе, урикозурический и противовоспалительный эффекты, позволяют рассматривать препараты иНГЛТ2 как приоритетные при СД2, ХБП и ХСН, в том числе в сочетании с подагрой [74-76]. Хорошая переносимость, отсутствие необходимости титровании дозы, возможность комбинации с основными СС препаратами наряду с клинически значимыми антиподагрическими свойствами — важные преимущества этой группы препаратов [<xref ref-type="bibr" rid="cit77">77</xref>]. Эти преимущества столь весомы, что для большинства пациентов с БГУ с прямыми показаниями к иНГЛТ2, эти препараты должны иметь приоритет перед УСТ.</p></sec><sec><title>Заключение</title><p>ГУ должна рассматриваться как один из триггеров воспалительной реакции, реализация которой находится в зависимости от целого ряда внешних и внутренних сопутствующих факторов. Рассмотрение аспектов этих сложных взаимодействий, ставших известными лишь недавно — одна из задач этой статьи, призванной акцентировать внимание на инфламмасоме — ключевой клеточной структуре воспалительного каскада. Это наиболее важно в отношении БГУ, при которой ингибиторы ксантиноксидазы до сих пор не имеют доказанной эффективности, что подчеркивает наличие терапевтических мишеней, более значимых для предотвращения кардиальных и почечных осложнений, чем ГУ.</p><p>Отношения и Деятельность. Нет.</p><p>Relationships and Activities. None.</p><p>1 Препарат не зарегистрирован в РФ.
